1. What is Tachidol and what is it used for
Tachidol contains two active ingredients (paracetamol and codeine), which work by relieving pain. The association
of paracetamol and codeine has a greater analgesic activity than that of its components taken
separately, with a more prolonged effect over time.
Tachidol is indicated in adultsfor the treatment of symptoms of moderate to severe pain that does not respond to
treatment with other analgesic medicines used alone.
The use of codeine (one of the two components of Tachidol) is indicated in patients of age over 12 years
for the treatment of acute pain (pain that occurs suddenly) of moderate intensity and that does not respond
adequately to other analgesics such as paracetamol or ibuprofen.
2. What you need to know before taking Tachidol
Do not take Tachidol
- if you are allergic to paracetamol or codeine or any of the other ingredients of this medicine (listed in section 6)
- if you are under 12 years of age
- if you suffer from severe hemolytic anemia (a condition due to the destruction of some blood cells, red blood cells)
- if you suffer from a severe liver disease (severe hepatocellular insufficiency)
- if you suffer from active liver disease
- if you suffer from a lung disease (respiratory failure)
- if you know that you have a liver enzyme, called CYP2D6, particularly active (ultra-rapid metabolizer for CYP2D6)
- if you are breastfeeding or are pregnant (see section 2 “Pregnancy and breastfeeding”).
In addition, patients between 0 and 18 years of age who have undergone surgery to remove their tonsils or adenoids due to a breathing disorder called obstructive sleep apnea syndrome should not take Tachidol, as, in such conditions, the risk of developing serious and life-threatening adverse reactions increases.
Also, do not take Tachidol granulesand Tachidol effervescent tabletsif you suffer from phenylketonuria (see sections “Tachidol effervescent granules contains maltitol, sodium and aspartame” and “Tachidol effervescent tablets contains sorbitol, sodium and aspartame”).
Warnings and precautions
Tolerance, physical dependence and psychological dependence
This medicine contains codeine which is an opioid medicine. Repeated use of opioids can lead to a decrease in the effectiveness of the drug (the body gets used to the drug in a phenomenon called tolerance). Repeated use of Tachidol can also cause physical dependence, abuse and psychological dependence, which can lead to a life-threatening overdose. If you take Tachidol for longer than recommended or at higher doses than recommended, the risk of these undesirable effects may increase and you will also be at risk of serious damage to your stomach/intestines and kidneys, as well as very low levels of potassium in the blood, which can be potentially fatal (see section 4).
Physical or psychological dependence can make you feel like you no longer have control over the amount of medicine you need to take or how often you need to take it.
The risk of physical or psychological dependence varies from person to person. You may be at higher risk of physical or psychological dependence on Tachidol if:
- you or a family member has abused or been dependent on alcohol, prescription drugs or illegal drugs (“psychological dependence”).
- you are a smoker.
- you have had mood disorders (depression, anxiety or personality disorder) in the past or have been treated by a psychiatrist for other mental illnesses.
If you notice any of the following signs while taking [product name], it could be a sign of physical or psychological dependence:
- you need to take the medicine for longer than recommended by your doctor.
- you need to take larger quantities than the recommended dose.
- you may feel the need to continue taking the medicine, even if it does not help to relieve your pain.
- you are using the medicine for reasons other than those prescribed, for example, ‘to stay calm’ or ‘to help you sleep’.
- you have made repeated attempts to stop or control the use of the medicine, without success.
- when you stop taking the medicine you do not feel well and you feel better when you take it again (‘withdrawal effects’). If you notice any of these signs, consult your doctor to agree on the best treatment plan for you, including when it will be appropriate to stop it and how to stop it safely (see section 3, If you stop taking Tachidol).
Talk to your doctor or pharmacist before taking Tachidol.
In the following cases, take Tachidol with caution:
- if you suffer from kidney disease (renal insufficiency)
- if you suffer from liver disease (hepatic insufficiency)
- if you consume alcohol chronically or consume it excessively (3 or more alcoholic beverages per day)
- if you suffer from anorexia (eating disorder characterized by lack or reduction of appetite)
- if you suffer from bulimia (eating disorder in which a person ingests an excessive amount of food and then resorts to various methods to eliminate it)
- if you suffer from cachexia (a condition characterized by extreme thinness, reduction of muscle mass and thinning of the skin due to chronic diseases)
- if you have been following an incorrect diet for a long time (chronic malnutrition)
- if you suffer from dehydration (severe loss of water/fluids from the body)
- if you suffer from hypovolemia (reduced blood volume in circulation)
- if you suffer from glucose-6-phosphate dehydrogenase deficiency (a substance normally present in the human body, the deficiency of which can cause a blood disease)
- if you have undergone surgery to remove the gallbladder. In this case you may have acute abdominal pain
- if you have a cough and produce phlegm, as expectoration may be difficult.
During treatment with Tachidol, immediately inform your doctor if:
If you suffer from serious illnesses, including severe kidney impairment or sepsis (when bacteria and their toxins circulate in the blood causing organ damage) or malnutrition, chronic alcoholism or if you are also taking flucloxacillin (an antibiotic). Patients in these situations have reported a serious condition called metabolic acidosis (an abnormality of the blood and fluids) when paracetamol is used at regular doses for a prolonged period or when paracetamol is taken in association with flucloxacillin. The symptoms of metabolic acidosis may include: severe difficulty breathing with rapid deep breathing, drowsiness, nausea and vomiting.
Sleep-related breathing disorders
Tachidol can cause sleep-related breathing disorders such as sleep apnea (pauses in breathing during sleep) and sleep-related hypoxemia (low level of oxygen in the blood). Symptoms may include pauses in breathing during sleep, waking up at night due to shortness of breath, difficulty staying asleep, or excessive daytime sleepiness. If you or another person experiences these symptoms, contact your doctor. The doctor may consider reducing the dose.
Contact your doctor if you experience severe pain in the upper abdomen that may radiate to the back, nausea, vomiting or fever, as these may be symptoms associated with inflammation of the pancreas (pancreatitis) and the biliary tract
Consult your doctor or pharmacist if you experience pain or increased sensitivity to pain (hyperalgesia) that does not respond to a higher dose of the medicine
The use of Tachidol must be discontinued at the first appearance of skin rash or any other sign of hypersensitivity (allergic reaction).
- respiratory depression (reduction of respiratory function) and sedation, lowering of the convulsive threshold (i.e., increased excitability of the nervous system, as in the case of epilepsy)
- hyperalgesia (increased sensitivity to pain)
- constipation (difficult or infrequent evacuation)
- nausea and vomiting
- masking of the symptoms of acute abdominal conditions (conditions characterized by severe pain and signs of irritation of the abdomen)
- obstruction of the outflow of bile and pancreatic juice, as in the case of some diseases of the biliary tract, for example pancreatitis (inflammation of the pancreas) and cholelithiasis (presence of gallstones in the gallbladder)
- increased release of histamine, as in the case of asthma (bronchial obstruction)
- pruritus
- reduction of hormone levels
- inhibition of immune function (i.e., function of the body's defense system)
- muscle rigidity and myoclonus (rapid involuntary muscle twitch)
- urinary retention (difficulty emptying the bladder) particularly in patients with narrowing of the urethra (the last tract of the urinary tract) or with enlargement of the prostate.
Use of high doses and/or for prolonged periods
The use of high doses and/or prolonged use of this medicine can cause liver disorders (hepatopathy), serious alterations to the kidneys and blood or lead to dependence, i.e. a situation in which you cannot do without the drug.
In these cases, the doctor will monitor the functionality of the liver, kidneys and blood composition over time through appropriate examinations. Before taking this medicine, tell your doctor if the person who is going to take the medicine suffers from a kidney or liver disease.
Prolonged use of analgesics, including opioids, increases the risk of headache (headache) from drug abuse.
With prolonged use of opioids, a reduction in analgesic efficacy (tolerance) may occur.
Prolonged administration of codeine or at doses higher than therapeutic ones can lead to physical and psychological dependence (habituation) and withdrawal syndrome following sudden discontinuation of administration, which can be observed in both patients and infants born to codeine-dependent mothers.
Tachidol should be used with caution in patients with dependence or with a history of dependence on opioids, and alternative analgesic therapies should be considered.
If you are elderly
If you are elderly and suffer from an illness (e.g. to the brain, bladder, etc.), prolonged use of the medicine may worsen the illness.
Elderly patients may be at greater risk of adverse effects associated with opioid use, such as respiratory depression and constipation.
Children and adolescents
Donot give this medicine to children under 12 years of age.
Donot give Tachidol to relieve pain in children and adolescents after removal of the tonsils or adenoids for obstructive sleep apnea syndrome.
Donot give Tachidol to children and adolescents with breathing problems, as the symptoms of morphine toxicity may be worse.
Other medicines and Tachidol
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other
medicine.
In particular, tell your doctor if the person taking the medicine is taking:
- medicines that contain paracetamol.During treatment with paracetamol, before taking any other medicine, check that it does not contain paracetamol, as if paracetamol is taken in high doses, serious side effects may occur;
- medicines that speed up stomach emptying(e.g. prokinetics), as these medicines speed up the effect of paracetamol;
- cholestyramine(a medicine used to reduce cholesterol levels in the blood), as it reduces the effect of paracetamol;
- chloramphenicol(an antibiotic), as it may increase the risk of side effects;
- medicines or substances that in chronic treatment alter liver function, for example:
- rifampicin (an antibiotic)
- cimetidine (a medicine used in the treatment of stomach ulcers)
- antiepileptics (medicines used to treat epilepsy), for example glutethimide, phenobarbital, carbamazepine, phenytoin.
- zidovudine (medicine for the treatment of HIV). The use of these medicines with Tachidol requires close medical monitoring.
- medicines or substances that prolong the effect of paracetamol, for example:
- probenecid (a medicine used in the treatment of gout)
- salicylamide (an anti-inflammatory).
- oral anticoagulantswhich are medicines used to make the blood more fluid, in this case the doctor will reduce the dose of Tachidol
- medicines that alter the activity of the central nervous systemsuch as:
- sedatives and tranquilizers
- antihistamines, medicines used to treat allergies
- neuroleptics, medicines to treat mental illnesses
- meprobamate, a medicine to treat anxiety
- hypnotics, medicines to treat insomnia
- medicines to treat depression (amitriptyline, doxepin, mirtazapine, mianserin, trimipramine)
- central-acting antihypertensives, medicines to treat high blood pressure
- benzodiazepines
- barbiturates
- gabapentin or pregabalin for the treatment of epilepsy or pain originating at the nerve level (neuropathic pain)
- analgesics(alfentanil, destromoramide, dextropropoxyphene, fentanyl, dihydrocodeine, hydromorphone, morphine, oxycodone, pethidine, phenoperidine, remifentanil, sufentanil, tramadol).
- medicines to treat cough(dextromethorphan, noscapine, folcodine, codeine, ethimorphine).
- methadone,a medicine to treat drug addiction.
- baclofen, medicine to reduce and relieve excessive tension in the muscles (spasms) that occurs in various diseases.
- thalidomide, a medicine to treat a disease of the immune system, the body's defense system (multiple myeloma).
- anticholinergics(medicines to treat various conditions such as bladder problems, allergies and depression);
- flucloxacillin(antibiotic), due to a serious risk of blood and fluid abnormality (called metabolic acidosis with high anion gap) which needs to be treated urgently.
Do nottake Tachidol if you are taking:
- potent analgesics(buprenorphine, nalbuphine, pentazocine)
- naltrexone, a medicine to treat drug addiction.
The concomitant use of Tachidol and sedative medicines such as benzodiazepines or related drugs increases the
risk of drowsiness, difficulty breathing (respiratory depression), coma and can be life-threatening. For this reason, concomitant use should only be considered when other
therapeutic options are not possible.
However, if your doctor prescribes Tachidol together with sedative medicines, they must limit the dose and duration
of the concomitant treatment.
Tell your doctor about all the sedative medicines you are taking and strictly follow their
recommendation on the dose. It may be helpful to inform friends and family to be aware of the signs and symptoms
reported above. Contact your doctor when these symptoms occur.
Tachidol and alcohol
Do not take Tachidol with alcoholic beverages, as alcohol increases sedation and causes a
state of reduced alertness that can make driving and operating machinery dangerous (see section “Driving and
operating machinery”).
Take Tachidol with caution if you consume alcohol chronically or excessively (3 or more alcoholic beverages
per day), as there is a risk of intoxication (see sections “Warnings and precautions” and “If you take more Tachidol than you should”).
Pregnancy and breastfeeding
If you are pregnant, suspect you may be pregnant or are planning a pregnancy, ask your doctor or
pharmacist for advice before taking this medicine.
Pregnancy
Do nottake Tachidol during pregnancy (see section “Do not take Tachidol”).
Use in the third trimester may be associated with the possible occurrence in the newborn of opioid withdrawal symptoms including restlessness, excessive crying, tremors, increased muscle tone (hypertonia), increased
respiratory rate (tachypnea), fever (pyrexia), vomiting and diarrhea.
Breastfeeding
Do nottake Tachidol if you are breastfeeding as codeine passes into breast milk (see section
“Do not take Tachidol”).
Driving and operating machinery
Tachidol may affect your ability to drive vehicles or operate machinery as it may cause drowsiness and
reduced alertness.
Tachidol effervescent granules contains maltitol, sodium and aspartame
Maltitol: if your doctor has diagnosed you with an intolerance to certain sugars, contact them before taking this
medicine.
Sodium: this medicine contains 12.3 mmol (or 282.9 mg) of sodium (the main component of table salt)
per sachet. This is equivalent to 14% of the recommended maximum daily intake with the diet of an adult.
Aspartame: this medicine contains 25 mg of aspartame per dose equivalent to 25 mg/5945 mg. Aspartame
is a source of phenylalanine. It may be harmful if you have phenylketonuria, a rare genetic disorder that
causes a build-up of phenylalanine because the body cannot properly dispose of it (see section “Do not
take Tachidol”).
Tachidol effervescent tablets contains sorbitol, sodium and aspartame
Sorbitol: this medicine contains 252 mg of sorbitol per tablet equivalent to 252 mg/3000 mg.
Sodium: this medicine contains 15.2 mmol (or 349.6 mg) of sodium (the main component of table salt)
per effervescent tablet.
This is equivalent to 17.5% of the recommended maximum daily intake with the diet of an adult.
Aspartame: this medicine contains 30 mg of aspartame per effervescent tablet equivalent to 30
mg/3000 mg. Aspartame is a source of phenylalanine. It may be harmful if you have phenylketonuria, a rare
genetic disorder that causes a build-up of phenylalanine because the body cannot properly dispose of it (see
section “Do not take Tachidol”).
Tachidol film-coated tablets contains lactose
If your doctor has diagnosed you with an intolerance to certain sugars, contact them before taking this
medicine.
3. How to take Tachidol
Take this medicine following exactly the instructions of your doctor or pharmacist. If you have any doubts
consult your doctor or pharmacist.
Adults and adolescents over 12 years of age
Effervescent granules in sachets
The recommended dose is 1-2 sachets as needed, up to 3 times a day.
Dissolve Tachidol effervescent granules (sachets) in half a glass of water and take it at intervals of at least
4 hours.
Effervescent tablets
The recommended dose is 1-2 effervescent tablets as needed, up to 3 times a day.
Take Tachidol effervescent tablets at intervals of at least 4 hours.
Dissolve the effervescent tablets in a glass of water.
Film-coated tablets
The recommended dose is 1-2 tablets as needed, up to 3 times a day.
Take Tachidol tablets at intervals of at least 4 hours.
Before starting treatment and regularly during treatment, your doctor will explain what you can expect
from the use of Tachidol when and for how long you should take it, when to see a doctor and when you should
stop it (see also, If you stop taking Tachidol).
If you suffer from renal or hepatic insufficiency (severe reduction in kidney or liver function)
If your doctor has diagnosed you with severe renal or hepatic insufficiency, the interval between administrations must
be at least 8 hours.
Duration of treatment
Do not take Tachidol for more than 3 consecutive days without consulting your doctor. The doctor must assess the
need for treatments for more than 3 consecutive days.
If you take more Tachidol than you should
In case of accidental ingestion/intake of an excessive dose of Tachidol, immediately inform
your doctor or go to the nearest hospital. In the hospital, you will be subjected to appropriate intervention protocols (e.g.
gastric lavage, administration of the antidote).
In case of accidental intake of very high doses of Tachidol, the symptoms you may experience
related to paracetamol are: anorexia (lack or reduction of appetite), nausea, vomiting, pallor,
abdominal pain, malaise, sweating. Paracetamol, taken in doses far exceeding those
commonly recommended, can cause serious liver damage that can progress to coma and death.
In adults, an overdose of codeine can cause: decreased breathing, skin that turns blue due to
lack of oxygen (cyanosis), drowsiness, flushing of the skin, itching, vomiting, difficulty with movements
(ataxia). In children, an overdose of codeine can cause: decreased or stopped breathing,
constriction of the pupils (miosis), convulsions, facial redness and swelling, hives, collapse, urinary retention.
In order to avoid the risk of overdose or serious side effects, check that other medications
taken (with or without a prescription) do not contain opioids or other central nervous system depressants.
Taking an excessive dose of Tachidol increases the risk of poisoning and can lead to death,
especially in the following cases:
- if the person taking the drug suffers from liver diseases
- if the person taking the drug chronically abuses alcohol (chronic alcoholism)
- if the person taking the drug has been following an incorrect diet for a long time (chronic malnutrition)
- if the person taking the drug is taking medications or substances that increase liver function.
If you forget to take Tachidol
Do not take/give an adolescent a double dose to make up for a missed dose.
If you stop taking Tachidol
If you take Tachidol at doses higher than those therapeutic, you are at risk of dependence and withdrawal syndrome
following sudden discontinuation of administration. These effects may also be visible on
a newborn child if you took codeine for a prolonged period during pregnancy.
If you have any questions about the use of this medicine, ask your doctor or pharmacist.
4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody gets them.
If you experience the following side effects STOPtreatment with this medicine and contact
immediatelyyour doctor:
- slow or difficult breathing
- confusion
- drowsiness
- pinpoint pupils
- nausea or vomiting
- constipation
- loss of appetite
- allergic reactions such as for example:
- swelling of hands, feet, ankles, face, lips, tongue and/or throat (angioedema)
- swelling of the throat (laryngeal edema)
- severe allergic reaction (anaphylactic shock)
- itching
- redness of the skin accompanied by itching (urticaria)
- skin rashes (rash).
In addition, people taking this medicine may experience the following side effects.
Frequency not known(frequency cannot be estimated based on available data)
- skin reactions of various types and severity including cases of erythema multiforme, Stevens Johnson syndrome and epidermal necrolysis (skin diseases that manifest with red spots, blistering lesions with areas of skin detachment). Very rare cases of severe skin reactions have been reported
- decreased number of platelets in the blood (thrombocytopenia)
- decreased number of white blood cells in the blood (leukopenia/agranulocytosis/neutropenia)
- decreased hemoglobin in the blood, a substance that carries oxygen in the blood (anemia)
- excitement (euphoria)
- depressed mood with agitation, irritability, nervousness (dysphoria)
- hallucination
- drug abuse
- drug dependence
- myoclonus (rapid involuntary muscle twitch)
- paresthesia (tingling)
- syncope (transient loss of consciousness)
- tremor
- sedation
- liver inflammation (hepatitis)
- biliary colic (severe pain in the upper abdomen)
- decreased kidney function (acute renal failure)
- kidney inflammation (interstitial nephritis)
- blood in the urine (hematuria)
- cessation or severe reduction in urine production (anuria)
- inability to empty the bladder (urinary retention)
- stomach and intestinal disorders
- inflammation of the pancreas (pancreatitis)
- acute abdominal pain (which may occur particularly in patients who have undergone gallbladder removal)
- dizziness
- constriction of the muscles around the airways (bronchospasm)
- dyspnea (difficulty breathing).
- severe condition that can make the blood more acidic (called metabolic acidosis), in patients with a serious illness who use paracetamol (see paragraph 2).
At codeine dosages higher than therapeutic doses there is a risk of dependence and withdrawal syndrome following
sudden discontinuation of administration which can be observed in both patients and newborns
born to codeine-dependent mothers (see paragraph “If you stop taking Tachidol”).
In case of overdose, paracetamol can cause serious liver damage (cytolysis which can evolve
towards massive and irreversible necrosis).
Reporting of side effects
If you experience any side effects, including those not listed in this leaflet, please inform your
doctor or pharmacist. You can also report side effects directly through the website
http://www.aifa.gov.it/content/segnalazioni- reazioni-avvers e.
Reporting side effects can help provide more information on the safety of this
medicine.
Taking Tachidol may alter the results of some diagnostic tests:
- tests of liver function;
- uricemia;
- glycemia;
- decrease or increase of INR (value used to express the time it takes for blood to coagulate).
5. How to store Tachidol
Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the pack after EXP. The expiry date
refers to the last day of that month.
Store this medicine in a safe and secure place inaccessible to others. It may cause
serious harm and be fatal to people for whom it is not intended.
Effervescent granules in sachets: store at a temperature below 25 C.
Effervescent tablets: store at a temperature below 30 C. Store in the original packaging to
protect the medicine from moisture. Shelf life after first opening of the tube: use the medicine within 3 months;
any excess product must be discarded.
Film-coated tablets: this medicine does not require any special storage conditions.
A half tablet can be stored for 24 hours in the original packaging to protect it from light.
Do not dispose of any medicine in wastewater or household waste. Ask your pharmacist how to dispose of the
medicines you no longer use. This will help protect the environment.
6. Package Contents and Other Information
What Tachidol Contains
Tachidol 500 mg/30 mg effervescent granules
- the active ingredients are: paracetamol and codeine (each sachet contains 500 mg of paracetamol and 30 mg of codeine phosphate
- the other ingredients are: mannitol, maltitol, sodiumbicarbonate, sodiumdocusate, aspartame(see paragraph “Tachidol effervescent granules contains maltitol, sodium and aspartame”), citric acid, lemon flavour, povidone.
Tachidol 500 mg/30 mg effervescent tablets
- the active ingredients are: paracetamol and codeine (each effervescent tablet contains 500 mg of paracetamol and 30 mg of codeine phosphate)
- the other ingredients are: anhydrous citric acid, sorbitol, sodiumbicarbonate, anhydrous sodiumcarbonate, sodiumdocusate, aspartame(see paragraph “Tachidol effervescent tablets contains sorbitol, sodium and aspartame”), orange flavour, lemon flavour, simethicone emulsion.
Tachidol 500 mg/30 mg film-coated tablets
- the active ingredients are: paracetamol and codeine (each tablet contains 500 mg of paracetamol and 30 mg of codeine phosphate)
- the other ingredients are: sodium croscarmellose, stearic acid, povidone, precipitated silica, sodium lauryl sulfate, lactosemonohydrate (see paragraph “Tachidol film-coated tablets contains lactose”), hypromellose, titanium dioxide (E171), Macrogol 4000.
Description of the appearance of Tachidol and package contents
Tachidol effervescent granules
It is presented in the form of white granules contained in aluminium paper sachets and packaged in cartons
of 10 bipartite sachets.
Tachidol effervescent tablets
They are presented in the form of effervescent tablets, white or almost white, round and with a fracture line on
one side.
They are packaged in cartons of 16 tablets contained in two plastic tubes, each of which contains 8
divisible effervescent tablets.
Tachidol film-coated tablets
They are presented in the form of white-whitish tablets, film-coated, biconvex and oblong, with a fracture line on
one side.
They are packaged in opaque blisters and available in packs of 10, 12, 16, 20, 24 tablets.
Not all pack sizes may be marketed.
