1. What Enalapril and Hydrochlorothiazide Aurobindo is and what it is used for

Enalapril and Hydrochlorothiazide Aurobindo contains two active ingredients: enalapril maleate and hydrochlorothiazide.
Enalapril maleate belongs to a group of medicines known as angiotensin converting enzyme inhibitors (ACE inhibitors). ACE inhibitors work by relaxing blood vessels, and therefore facilitating the passage of blood through them. ACE inhibitors are used for the treatment of high blood pressure (hypertension).
Hydrochlorothiazide belongs to a group of medicines called thiazide diuretics. It works by stimulating the kidneys to produce a greater quantity of urine (diuretic action) and thereby reducing blood pressure (antihypertensive action).
Enalapril and Hydrochlorothiazide Aurobindo is used for the treatment of high blood pressure and is indicated in patients whose blood pressure is not adequately controlled with enalapril maleate or hydrochlorothiazide administered alone.

2. Cosa deve sapere prima di prendere Enalapril e Idroclorotiazide Aurobindo

Non prenda Enalapril e Idroclorotiazide Aurobindo:

  • se è allergico ai principi attivi (enalapril maleato, idroclorotiazide) o ad altri medicinali derivati dalla sulfonamide (sostanza chimicamente correlata all’idroclorotiazide) o ad uno qualsiasi degli altri componenti di questo medicinale (elencati al paragrafo 6);
  • se ha avuto una reazione allergica che ha provocato gonfiore delle mani, dei piedi, delle caviglie, o anche del viso, delle labbra, della lingua e/o della gola con conseguente difficoltà a deglutire o respirare (angioedema) dopo l’assunzione di ACE-inibitori;
  • se soffre di angioedema ereditario (dovuto a predisposizione familiare);
  • se soffre di angioedema idiopatico, ovvero angioedema la cui causa non è nota;
  • se soffre di gravi problemi ai reni (insufficienza renale grave);
  • se non riesce ad urinare (anuria);
  • se è al secondo e terzo trimestre di gravidanza (vedere il paragrafo “Gravidanza e allattamento”);
  • se soffre di gravi problemi al fegato (insufficienza epatica grave);
  • se soffre di diabete o la sua funzione renale è compromessa ed è in trattamento con un medicinale che abbassa la pressione del sangue, contenente aliskiren (vedere il paragrafo “Altri medicinali ed Enalapril e Idroclorotiazide Aurobindo”);

  • sta assumendo medicinali che aumentano la produzione di urina (diuretici), sta seguendo una dieta povera di sale, oppure se ha avuto vomito o diarrea. In questi casi lei è a rischio di un eccessivo calo della pressione sanguigna (ipotensione) che può manifestarsi con sensazione di debolezza o testa leggera. Il medico le prescriverà di misurare regolarmente la quantità di elettroliti presenti nel sangue;
  • presenta disturbi del flusso sanguigno del cuore (cardiopatia ischemica) o del cervello (cerebropatia ischemica), o altri problemi al cuore associati o meno a problemi ai reni. In questi casi un abbassamento eccessivo della pressione del sangue può risultare particolarmente pericoloso. Se manifesta i sintomi di ipotensione durante il trattamento con Enalapril e Idroclorotiazide Aurobindo si distenda e contatti il medico;
  • ha avuto in passato il cancro della pelle o se sta sviluppando una lesione della pelle imprevista durante il trattamento. Il trattamento con idroclorotiazide, in particolare un utilizzo a lungo termine con dosi elevate, può aumentare il rischio di alcuni tipi di cancro della pelle e delle labbra (cancro della pelle non melanoma). Protegga la sua pelle dall’esposizione al sole e ai raggi UV durante l’assunzione di Enalapril e Idroclorotiazide Aurobindo;
  • sta assumendo un medicinale contenente un inibitore della neprilisina come sacubitril (disponibile come associazione a dose fissa con valsartan), usato nei pazienti con insufficienza cardiaca;
  • racecadotril, un medicinale usato per trattare la diarrea;
  • medicinali usati per prevenire il rigetto di organi trapiantati e per trattare il cancro (come temsirolimus, sirolimus, everolimus);
  • vildagliptin, un medicinale usato per trattare il diabete;
  • un “antagonista del recettore dell’angiotensina II” (AIIRA) (anche noti come sartani - per esempio valsartan, telmisartan, irbesartan), in particolare se soffre di problemi renali correlati al diabete;
  • aliskiren. Il medico può controllare la sua funzione renale, la pressione del sangue e la quantità di elettroliti (ad esempio il potassio) nel sangue a intervalli regolari. Vedere anche quanto riportato al paragrafo “Non prenda Enalapril e Idroclorotiazide Aurobindo”;
  • insufficienza renale, peggioramento della funzione renale, età superiore a 70 anni, diabete, disidratazione, scompenso cardiaco acuto a causa del quale il cuore non pompa adeguatamente il sangue in tutto il corpo, aumento della quantità degli acidi nel sangue (acidosi metabolica);
  • assunzione concomitante di medicinali che facilitano l’eliminazione delle urine preservando il potassio (diuretici risparmiatori di potassio, per esempio spironolattone, eplerenone, triamterene o amiloride), integratori di potassio, sostituti del sale contenenti potassio, o se sta assumendo altri medicinali che possono aumentare il potassio nel sangue (ad es. eparina - un medicinale utilizzato per prevenire i coaguli di sangue, prodotti contenenti trimetoprim come cotrimoxazolo - medicinali utilizzati per il trattamento di infezioni) (vedere paragrafo “Altri medicinali e Enalapril e Idroclorotiazide Aurobindo). In tali casi il medico monitorerà frequentemente il livello di potassio nel sangue;
  • soffre di un restringimento (stenosi) dell’arteria aortica o ha un ispessimento delle pareti del cuore (cardiomiopatia ipertrofica);
  • soffre di gravi problemi al cuore (insufficienza cardiaca grave);
  • ha problemi ai reni, ad esempio un restringimento (stenosi) di una o entrambe le arterie renali, in quanto esiste un rischio aumentato di ipotensione e insufficienza renale. In tal caso il medico la terrà sotto stretta osservazione, monitorando la sua funzione renale;
  • si è recentemente sottoposto ad un trapianto di reni. In questo caso l’uso di questo medicinale non è raccomandato;
  • è sottoposto a dialisi (terapia depurativa del sangue) a causa di insufficienza renale. In questo caso il medico prenderà in considerazione un trattamento alternativo (vedere paragrafo “Non prenda Enalapril e Idroclorotiazide Aurobindo”);
  • ha problemi al fegato o soffre di una malattia che potrebbe portare ad una distruzione delle cellule del fegato (malattia epatica progressiva) (vedere paragrafi “Non prenda Enalapril e Idroclorotiazide Aurobindo” e “Sospenda il trattamento con Enalapril e Idroclorotiazide Aurobindo ed informi il medico se”);
  • ha una malattia cronica e degenerativa del fegato (cirrosi epatica), presenta una produzione marcata di urina, non assume una quantità adeguata di elettroliti per via orale, è in trattamento con corticosteroidi o ormone adrenocorticotropo (medicinali ormonali). In tali casi il rischio di un abbassamento del livello di potassio del sangue (ipokaliemia) è massimo;
  • soffre di una malattia del tessuto connettivo che coinvolge i vasi sanguigni, sta assumendo medicinali che sopprimono le difese immunitarie (immunosoppressori), soffre di gotta o sta assumendo allopurinolo (usato nel trattamento della gotta) o procainamide (usata per trattare anomalie del ritmo cardiaco), o presenta una combinazione di questi fattori di rischio, in particolare se presenta anche problemi ai reni. In questo caso si possono manifestare gravi infezioni che potrebbero non rispondere alla terapia con antibiotici. In caso lei debba assumere Enalapril e Idroclorotiazide Aurobindo in presenza di queste condizioni, il medico le prescriverà specifici esami del sangue. Riferisca al medico qualsiasi segno o sintomo di infezione;
  • ha il diabete e sta assumendo medicinali antidiabetici assunti per via orale o insulina. In tal caso il medico le dirà di effettuare esami del sangue per controllare il livello di glucosio, in particolare durante il primo mese di trattamento con Enalapril e Idroclorotiazide Aurobindo, e valuterà la necessità di adeguare la dose dei medicinali antidiabetici;
  • sta assumendo litio (medicinale per il trattamento di disturbi dell’umore) (vedere paragrafo “Altri medicinali e Enalapril e Idroclorotiazide Aurobindo”);
  • è di razza nera. Enalapril e Idroclorotiazide Aurobindo può essere infatti meno efficace nel diminuire la pressione sanguigna nei pazienti di razza nera rispetto ai pazienti di razza non nera. Inoltre, potrebbe manifestare un rischio maggiore di sviluppare angioedema;
  • ha sviluppato tosse secca persistente per un lungo periodo di tempo dopo avere iniziato il trattamento con un ACE-inibitore;
  • ha precedenti di allergie o asma bronchiale, in quanto possono manifestarsi reazioni di sensibilità con l’uso di Enalapril e Idroclorotiazide Aurobindo;
  • soffre di un disturbo denominato lupus eritematoso sistemico (una malattia autoimmune), che può insorgere o peggiorare con l’uso di Enalapril e Idroclorotiazide Aurobindo;
  • ha una diminuzione della vista o dolore agli occhi. Questi potrebbero essere sintomi dell’accumulo di liquido nello strato vascolare dell’occhio (effusione coroidale) o di un aumento della pressione nell’occhio e possono verificarsi entro poche ore o settimane dopo l’assunzione di Enalapril e Idroclorotiazide Aurobindo;

Idroclorotiazide Aurobindo:

  • qualsiasi intervento chirurgico, o se deve essere sottoposto a terapia anestetica;
  • un trattamento chiamato aferesi delle LDL (lipoproteine a bassa densità) con destran-solfato, per rimuovere il colesterolo dal sangue per mezzo di macchinari. Il medico può interrompere il trattamento con Enalapril e Idroclorotiazide Aurobindo per prevenire una possibile reazione allergica;

interrompere temporaneamente la terapia;

  • gonfiore delle mani, dei piedi, delle caviglie, o anche del viso, delle labbra, della lingua e/o della gola con conseguente difficoltà a deglutire o respirare (angioedema);
  • colorazione gialla della pelle e della parte bianca degli occhi (ittero), che denotano problemi al fegato o un aumento dei livelli di transaminasi (enzimi del fegato) nel sangue;
  • aumento del livello di azoto e di creatinina nel sangue. Questa circostanza potrebbe far pensare alla possibilità di una stenosi dell’arteria renale;

Other medicines and Enalapril and Hydrochlorothiazide Aurobindo
Tell your doctor or pharmacist if you are taking, have recently taken, or might take any
other medicine.
In particular, tell your doctor or pharmacist if you are taking, have recently taken, or might
take any of the following medicines:

  • an angiotensin II receptor blocker (AIIRA) or aliskiren (see also what is reported in the paragraphs “Do not take Enalapril and Hydrochlorothiazide Aurobindo” and “Warnings and precautions”). Your doctor may need to adjust the dose and/or take other precautions;
  • other medicines that lower blood pressure (antihypertensives), including vasodilators, nitroglycerin and other nitrates, because an excessive reduction in blood pressure may occur;
  • lithium, used for the treatment of some mood disorders, because an excessive increase in lithium concentration in the blood and subsequent lithium toxicity may occur;
  • potassium supplements (including salt substitutes), potassium-sparing diuretics and other medicines that may increase the amount of potassium in the blood (such as trimethoprim and cotrimoxazole for treating bacterial infections; cyclosporine, an immunosuppressant used to prevent rejection of transplanted organs; and heparin, a medicine used to thin the blood and prevent the formation of clots);
  • medicines that promote urine excretion (diuretics), such as thiazides or loop diuretics, because they may cause a reduction in blood pressure;
  • anaesthetics, antipsychotics and tricyclic antidepressants (used for the treatment of mental disorders), because a further reduction in blood pressure may occur;
  • medicines that act on the autonomic nervous system (sympathomimetics), such as ephedrine, pseudoephedrine, salbutamol, which are found in some nasal decongestants, cough and cold medicines, for asthma, for the treatment of low blood pressure and for the

treatment of heart problems. These medicines can reduce the antihypertensive effects of
Enalapril and Hydrochlorothiazide Aurobindo;

  • insulin or other antidiabetic medicines taken by mouth. The blood glucose level should be carefully monitored, especially during the first weeks of treatment with Enalapril and Hydrochlorothiazide Aurobindo and in patients with kidney problems;
  • medicines used for the treatment of stiffness and inflammation associated with painful conditions, in particular those affecting muscles, bones and joints, including:
  • medicines containing gold administered by injection, for example sodium aurothiomalate, because they can cause flushing of the face, feeling unwell (nausea), vomiting and low blood pressure, if taken with Enalapril and Hydrochlorothiazide Aurobindo;
  • non-steroidal anti-inflammatory drugs (NSAIDs), including COX-2 inhibitors, because they can reduce the effectiveness of Enalapril and Hydrochlorothiazide Aurobindo, increase the potassium level in the blood and worsen kidney function;
  • carbenoxolone (medicine used to treat stomach problems such as ulcers), kaliuretic diuretics (for example furosemide) or excessive use of laxatives. The use of Enalapril and Hydrochlorothiazide Aurobindo with these medicines may further reduce potassium and/or magnesium levels in the blood;
  • tubocurarine, used to relax muscles during surgical operations (muscle relaxant), because Enalapril and Hydrochlorothiazide Aurobindo may enhance its effects;
  • barbiturates, which have a sedative effect on the central nervous system, used for example to treat epilepsy, or opioid analgesics, medicines used to reduce pain. These medicines can worsen the feeling of dizziness when getting up, due to a lowering of blood pressure (orthostatic hypotension);
  • medicines used to reduce the amount of fat in the blood, such as cholestyramine or colestipol resins, because they can reduce the absorption of Enalapril and Hydrochlorothiazide Aurobindo;
  • medicines used to treat irregular heartbeat (antiarrhythmics such as quinidine, procainamide, amiodarone, sotalol), because they can cause heart rhythm problems (torsade de pointes);
  • medicines used for the treatment of heart failure (digitalis medicines);
  • corticosteroids (which modulate anti-inflammatory reactions) and adrenocorticotropic hormone ACTH (used to check if the adrenal glands are functioning correctly), because they can further reduce salt levels in the blood, in particular potassium (hypokalaemia);
  • pressor amines (for example noradrenaline), because Enalapril and Hydrochlorothiazide Aurobindo can decrease their effects;
  • medicines for the treatment of cancer, such as cyclophosphamide or methotrexate, because Enalapril and Hydrochlorothiazide Aurobindo can increase their toxicity;
  • medicines to treat gout, such as allopurinol (see paragraph “Warnings and precautions”);
  • medicines that suppress the immune defenses (immunosuppressants);
  • mTOR inhibitors (e.g. temsirolimus, sirolimus, everolimus), as concomitant administration may increase the risk of an allergic reaction called angioedema (see the paragraph “Stop taking Enalapril and Hydrochlorothiazide Aurobindo and tell your doctor if”);
  • a medicine containing a neprilysin inhibitor such as sacubitril (available as a fixed-dose combination with valsartan) and racecadotril. The risk of angioedema (swelling of the face, lips, tongue or throat with difficulty swallowing or breathing) may be increased. See also the information contained in the paragraphs “Do not take Enalapril and Hydrochlorothiazide Aurobindo” and “Warnings and precautions”.

Enalapril and Hydrochlorothiazide Aurobindo with alcohol
Enalapril and Hydrochlorothiazide Aurobindo should not be taken with alcohol, as an excessive
lowering of blood pressure may occur.

Pregnancy and breastfeeding
If you are pregnant, suspect you are pregnant, or are planning a pregnancy, or if you are breastfeeding with
breast milk, ask your doctor or pharmacist for advice before taking this medicine.

Pregnancy
Enalapril and Hydrochlorothiazide Aurobindo is not recommended during pregnancy, and should not be
taken if you are pregnant for more than 3 months, as it can cause serious harm to the baby if taken
after the third month of pregnancy (see the paragraph “Do not take Enalapril and Hydrochlorothiazide Aurobindo”).
You should tell your doctor if you think you are pregnant (or are planning a pregnancy).
Your doctor will tell you to stop taking Enalapril and Hydrochlorothiazide Aurobindo
immediately before starting a pregnancy or as soon as you find out you are pregnant, and will
advise you to take another medicine instead of Enalapril and Hydrochlorothiazide Aurobindo.

Breastfeeding
Tell your doctor if you are breastfeeding or intend to breastfeed your baby.
Enalapril and Hydrochlorothiazide Aurobindo is not recommended in women who are breastfeeding, in particular
if your baby was born prematurely, and in the first weeks after delivery.

Driving and operating machinery
Enalapril and Hydrochlorothiazide Aurobindo can cause side effects that may affect your ability
to drive vehicles and operate machinery, such as dizziness and fatigue that may occur
occasionally. Therefore, be careful when driving and operating machinery.

Enalapril and Hydrochlorothiazide Aurobindo contains lactose monohydrate
If your doctor has diagnosed you with intolerance to some sugars, contact him before taking this
medicine.

Enalapril and Hydrochlorothiazide Aurobindo and sodium
This medicine contains less than 1 mmol (23 mg) of sodium per tablet, i.e. it is essentially ‘sodium-free’.

For athletes
The use of the medicine without therapeutic need constitutes doping and may still result in positive
doping tests.

3. How to take Enalapril and Hydrochlorothiazide Aurobindo

Take this medicine following exactly the instructions of your doctor or pharmacist. If you have
doubts, consult your doctor or pharmacist.
It is advisable to start therapy with half a tablet a day. Subsequently, the recommended dose is
1 tablet a day. If necessary, your doctor may increase the dose up to a maximum of 2
tablets, administered once a day.

Previous diuretic therapy
If you are already taking another medicine that promotes diuresis, the use of enalapril, contained in
Enalapril and Hydrochlorothiazide Aurobindo, may determine an excessive reduction in blood
pressure. In such cases, if the doctor still deems the use of Enalapril and Hydrochlorothiazide
Aurobindo necessary, they will tell you to suspend the diuretic a few days before taking Enalapril and Hydrochlorothiazide
Aurobindo. If this is not possible, the doctor will decide to start therapy with lower doses of enalapril,
that is, with separate tablets of the same active ingredients as Enalapril and Hydrochlorothiazide Aurobindo (enalapril and
hydrochlorothiazide). Enalapril and Hydrochlorothiazide Aurobindo may then be used when the doctor
has established for you the need to take the dose of enalapril and hydrochlorothiazide present in the
tablet of Enalapril and Hydrochlorothiazide Aurobindo.

Patients with kidney problems (renal insufficiency)
You must not take Enalapril and Hydrochlorothiazide Aurobindo if you suffer from severe kidney problems (see the
section “Do not take Enalapril and Hydrochlorothiazide Aurobindo”). If you have kidney problems, your doctor
will check the function of your kidneys and decide the appropriate dose of Enalapril and Hydrochlorothiazide
Aurobindo.

Use in children and adolescents
The use of Enalapril and Hydrochlorothiazide Aurobindo is not recommended in children.

Use in the elderly
No dosage adjustments are necessary in the elderly.

If you take more Enalapril and Hydrochlorothiazide Aurobindo than you should
In case of accidental intake of an excessive dose of Enalapril and Hydrochlorothiazide Aurobindo, immediately warn
your doctor or go to the nearest hospital and stop treatment.
The symptoms of an overdose may include: feeling of emptiness or dizziness, due to an
excessive lowering of blood pressure, serious circulation problems (circulatory shock),
renal insufficiency, hyperventilation, increased heart rate (tachycardia), palpitations,
decreased heart rate (bradycardia), anxiety, cough, electrolyte disturbances such as
lowering of potassium (hypokalemia), chlorine (hypochloremia) and sodium (hyponatremia) in the
blood, dehydration due to excessive diuresis.

If you forget to take Enalapril and Hydrochlorothiazide Aurobindo
Do not take a double dose to make up for the missed dose. If in doubt, contact
your doctor.

If you stop taking Enalapril and Hydrochlorothiazide Aurobindo
Do not stop treatment with Enalapril and Hydrochlorothiazide unless your doctor tells you to.
If you have any doubts about the use of this medicine, ask your doctor or pharmacist.
