1. What is Topiramato Mylan Generics and what it is used for

Topiramato Mylan Generics belongs to a group of medicines called “antiepileptic drugs”.
It is used:

• alone for the treatment of epileptic seizures in adults and children over 6 years of age
• with other medicines for the treatment of epileptic seizures in adults and children aged 2 years and over
• to prevent migraine in adults.

2. What you need to know before you take Topiramato Mylan Generics

Do not take Topiramato Mylan Generics:

• if you are allergic to topiramate or any of the other ingredients of this medicine (listed in section 6). Migraine prevention
• You must not take Topiramato Mylan Generics if you are pregnant.
• If you are a woman who could become pregnant, you must not take Topiramato Mylan Generics unless you are using highly effective contraception (contraceptive) during treatment. See below under “Pregnancy, breastfeeding and fertility - Important warning for women”.

Epilepsy treatment

• You must not take Topiramato Mylan Generics if you are pregnant, unless no other treatment can offer you sufficient control of your seizures.
• If you are a woman who could become pregnant, you must not take Topiramato Mylan Generics unless you are using highly effective contraception (contraceptive) during treatment. The only exception is if Topiramato Mylan Generics is the only treatment that provides you with sufficient control of your seizures and you are planning a pregnancy. You should speak to your doctor to make sure you have received information about the risks of taking Topiramato Mylan Generics during pregnancy and the risks of seizures during pregnancy. See below under “Pregnancy, breastfeeding and fertility - Important warning for women”.

Make sure you have read the patient guide you will receive from your doctor
A patient alert card is provided inside the pack of Topiramato Mylan Generics to
remind you of the risks during pregnancy.
If you are not sure whether the conditions mentioned above apply to you, talk to your doctor or pharmacist before
using Topiramato Mylan Generics.

Warnings and precautions
Talk to your doctor before taking Topiramato Mylan Generics if you:

• have kidney problems, in particular:
• kidney stones
• high levels of calcium in your urine
• if a family member has or has ever had kidney stones
• or if you are undergoing kidney dialysis
• have a history of blood and fluid imbalances in the body (metabolic acidosis) which may be associated with:
• severe breathing problems
• severe or prolonged diarrhoea
• surgery
• or a diet high in fat and low in carbohydrates (ketogenic)
• have liver problems
• have eye problems, in particular glaucoma
• have growth problems
• are a woman who could become pregnant. Topiramato Mylan Generics can harm the fetus if taken during pregnancy. You must use highly effective contraception (contraceptive) during treatment and for at least 4 weeks after the last dose of Topiramato Mylan Generics. See section “pregnancy and breastfeeding” for further information.
• are pregnant. Topiramato Mylan Generics can harm the fetus if taken during pregnancy.

If you are not sure whether any of the above applies to you, talk to your doctor before taking Topiramato Mylan Generics.
If you have epilepsy, it is important that you do not stop taking the medicine without consulting your doctor first.
You should also talk to your doctor before taking any topiramate-containing medicine that is given to you
as an alternative to Topiramato Mylan Generics.
During treatment
During treatment with topiramate you may lose weight, so while taking this medicine, your weight
should be checked regularly. If you lose too much weight or if a child using the medicine does not
gain enough weight, consult your doctor.
A small number of people treated with antiepileptics like Topiramato Mylan Generics have experienced self-harmful or suicidal thoughts. If at any time you have these types of thoughts, contact
your doctor immediately.
Topiramate can cause serious skin reactions, tell your doctor immediately if you develop a rash
and/or blisters (see also section 4 'Possible side effects').
Topiramate can, in rare cases, cause high levels of ammonia in the blood (observed in blood tests) which
can lead to a change in brain function, especially if you are also taking a medicine
called valproic acid or sodium valproate. As this can be a serious condition, tell your
doctor immediately if you experience the following symptoms (See also section 4 “Possible side
effects”):

• difficulty thinking, remembering information, or solving problems
• being less attentive or aware
• feeling very sleepy with little energy. At higher doses of topiramate, the risk of developing these symptoms may increase. You may experience an increase in seizures (epilepsy) or seizures of a different type; in this case, consult your doctor. If you experience vision and/or eye problems, you should contact your doctor immediately. Other medicines and Topiramato Mylan GenericsTell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines. Topiramato Mylan Generics and some other medicines can affect each other. Sometimes the dose of some of the medicines you are taking or of Topiramato Mylan Generics will need to be adjusted. In particular, tell your doctor or pharmacist if you are taking:
• other medicines that impair or reduce thinking, concentration or muscle coordination (e.g., central nervous system depressants such as muscle relaxants and sedatives such as diazepam).
• hormonal contraceptives. Topiramato Mylan Generics can make hormonal contraceptives less effective. You must use an additional barrier method of contraception such as a condom or pessary/diaphragm. Talk to your doctor to discuss the best method of contraception to use while taking Topiramato Mylan Generics. Report any changes in your menstrual flow to your doctor while taking hormonal contraceptives and Topiramato Mylan Generics. Irregular menstrual flow may occur. In this case, continue taking your hormonal contraceptives and talk to your doctor. Keep a list of all the medicines you are taking. Show this list to your doctor or pharmacist before starting to take a new medicine. Other medicines to talk to your doctor or pharmacist about include other antiepileptic medicines, risperidone, lithium, hydrochlorothiazide, haloperidol (used to treat mental health problems), metformin, pioglitazone, glibenclamide (used to treat diabetes), amitriptyline, moclobemide, imipramine (used to treat

depression), propranolol, diltiazem, digoxin (used to treat heart disease), flunarizine (used to
treat migraine), proguanil (used to prevent malaria), omeprazole (used to treat digestive
problems), venlafaxine (a medicine used for depression) and St John's Wort ( Hypericumperforatum,
a herbal preparation used to treat depression), warfarin (used to thin the
blood).
If you are not sure whether any of the above applies to you, talk to your doctor or pharmacist before using
Topiramato Mylan Generics.

Topiramato Mylan Generics with alcohol
You can take Topiramato with or without food. During treatment with Topiramato Mylan Generics you should avoid
drinking alcohol.

Pregnancy, breastfeeding and fertility
Important information for women who may become pregnant
Topiramato Mylan Generics can harm the fetus. If you are a woman who may become pregnant, consult your
doctor to discuss other possible treatments. Consult your doctor to reassess the treatment and discuss the risks
at least once a year.

Prevention of migraine

• For migraine, you should not take Topiramato Mylan Generics if you are pregnant.
• For migraine, you should not take Topiramato Mylan Generics if you are a woman who may become pregnant unless you are using a highly effective contraceptive method.
• If the patient is a woman who may become pregnant, a pregnancy test must be performed before starting treatment with Topiramato Mylan Generics. Treatment of epilepsy
• For epilepsy, you should not take Topiramato Mylan Generics if you are pregnant, unless no other treatment can offer you sufficient control over seizures.
• For epilepsy, you should not take Topiramato Mylan Generics if you are a woman who may become pregnant unless you are using a highly effective contraceptive method. The only exception is if Topiramato Mylan Generics is the only treatment that provides you with sufficient control over seizures and you are planning a pregnancy. You should consult your doctor to make sure you have received information about the risks of taking Topiramato Mylan Generics during pregnancy and the risks of seizures during pregnancy, which may pose risks to you or the fetus.
• If the patient is a woman who may become pregnant, a pregnancy test must be performed before starting treatment with Topiramato Mylan Generics.

Risks associated with topiramate if taken during pregnancy (regardless of the disease for which

topiramate is used):
The use of Topiramato Mylan Generics during pregnancy carries risks for the fetus.

• If you take Topiramato Mylan Generics during pregnancy, your baby has a higher risk of developing congenital defects. Approximately 4-9 children out of 100 born to women taking topiramate have congenital defects. The number is 1-3 out of 100 for children born to women who do not suffer from epilepsy and do not take antiepileptic treatment. In particular, cleft lip (separation of the upper lip) and cleft palate (separation of the palate) have been observed. Male newborns may also present a malformation in the penis (hypospadias). These defects can develop early in pregnancy, even before you know you are pregnant.
• If you take Topiramato Mylan Generics during pregnancy, your baby may have a 2-3 times higher risk of developing autism spectrum disorders, intellectual disabilities, or attention deficit hyperactivity disorder (ADHD) compared to children born to women with epilepsy who do not take
  antiepileptic medications.

Use of Topiramato Mylan Generics in girls:
If you are a parent or guardian of a girl treated with Topiramato Mylan Generics, you must contact
your doctor immediately when the girl has her first menstrual period (menarche). The doctor will inform you about
the risks to the fetus caused by exposure to topiramate during pregnancy, and the need to use
a highly effective contraceptive.

If you want to become pregnant while taking Topiramato Mylan Generics:

• Make an appointment with your doctor.
• Do not stop using your contraceptive method without first talking to your doctor.
• If you are taking Topiramato Mylan Generics for epilepsy, do not stop taking it without first talking to your doctor, as your condition may worsen.
• Your doctor will reassess the treatment and consider alternative therapeutic options. Your doctor will inform you about the risks associated with Topiramato Mylan Generics during pregnancy. They may also recommend you to another specialist. If you become pregnant or suspect you are pregnant while taking Topiramato Mylan Generics:
• Make an urgent appointment with your doctor.
• If you are taking Topiramato Mylan Generics to prevent migraine, immediately stop taking the medicine and contact your doctor to assess the need for alternative treatments.
• If you are taking Topiramato Mylan Generics for epilepsy, do not stop taking this medicine without first talking to your doctor, as your condition may worsen. Worsening of epilepsy may pose risks to you or the fetus.
• Your doctor will reassess the treatment and consider alternative therapeutic options. Your doctor will inform you about the risks associated with Topiramato Mylan Generics during pregnancy. They may also recommend you to another specialist.
• If you take Topiramato Mylan Generics during pregnancy, you will need to be carefully monitored to check the development of the fetus. Make sure you have read the patient guide you will receive from your doctor. A patient alert card is provided inside the package of Topiramato Mylan Generics to remind you of the risks associated with topiramate during pregnancy. Driving and using machinesDizziness, fatigue and vision problems may occur during treatment with Topiramato Mylan Generics. Do not drive, or use tools or machinery, before talking to your doctor. Sodium contentThis medicine contains less than 1 mmol of sodium (23 mg) per film-coated tablet, i.e. essentially “sodium-free”. [Only 200 mg tablets]Content of sunset yellow aluminium lake (E110) and allura red aluminium lake (E129)This medicine contains sunset yellow aluminium lake (E110) and allura red aluminium lake (E129) which may cause allergic reactions.

3. How to take Topiramato Mylan Generics

Take this medicine following exactly the instructions of your doctor. Contact your doctor or pharmacist if you have any doubts.
Girls and women who could become pregnant:
Treatment with Topiramato Mylan Generics must be started and supervised by a doctor specializing in the
treatment of epilepsy or migraine. Consult your doctor to re-evaluate the treatment at least once
a year.

• Your doctor will usually give you a low dose of topiramate, then gradually increase it until the best dose for you is found.
• Topiramato Mylan Generics tablets must be swallowed whole. Avoid chewing the tablets as they may leave a bitter taste.
• Topiramato Mylan Generics can be taken before, during or after a meal. Drink plenty of fluids during the day, especially if you exercise, in case of high temperatures or if you do activities that expose you to heat to avoid the formation of kidney stones, while taking topiramate. Other formulations of this medicine are available that may be more suitable for patients who have difficulty swallowing tablets; ask your doctor or pharmacist about this. If you take more Topiramato Mylan Generics than you should
• Consult a doctor immediately. Bring the medicine packaging with you.
• You may feel sleepy, tired, or less alert; you may experience lack of coordination; have difficulty speaking or concentrating; see double or blurred; feel dizzy due to low blood pressure; feel depressed or agitated; have abdominal pain or sudden seizures (convulsions).
• If you are taking other medicines together with Topiramato Mylan Generics, an overdose may occur. If you forget to take Topiramato Mylan Generics
• If you forget to take a dose, take it as soon as you remember. However, if it is almost time for the next dose, skip the missed dose and continue as usual. If you forget two or more doses, contact your doctor.
• Do not take a double dose (two doses at the same time) to make up for the missed dose. If you stop taking Topiramato Mylan Generics

Do not stop taking this medicine unless your doctor tells you to. Your symptoms may
return. If your doctor decides to stop this medicine, your dose may be gradually reduced
over a few days.
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everyone will experience them.
Inform your doctor or seek immediate medical attention if you experience the following side effects:

Very common (may affect more than 1 in 10 people):

• Depression (new onset or worsening). Common (may affect up to 1 in 10 people):
• Convulsions (seizures)
• Anxiety, irritability, confusion, disorientation, changes in mood or behavior, including anger, nervousness, sadness
• Problems with concentration, slowness of thought, memory loss, memory problems (new onset, sudden change or increased severity)
• Kidney stones, frequent or painful urination, calcium deposits in the kidneys
• Allergic reaction (such as skin rash, redness, itching, and swelling of the face, including swelling of the nose, lips, eyelids and tongue, hives)
• Decreased vision. Uncommon (may affect up to 1 in 100 people):
• Increased number of infections with sore throat, fever, chills or ulcers in the mouth, which may be due to a decrease in the number of white blood cells in the blood detected by blood tests
• Increased level of acid in the blood (may cause difficulty breathing, including shortness of breath, loss of appetite, nausea, vomiting, excessive fatigue and fast or irregular heartbeats)
• Decreased or loss of sweating which can cause a serious increase in temperature with hot and dry skin, feeling or being unwell, headache, dizziness and fainting
• Thoughts of serious self-harm, attempts to cause serious self-harm
• Hearing, seeing or feeling things that are not there, severe mental disorders (psychosis)
• Loss of control of muscle movements that allow walking in a straight line and can be determined by medical tests
• Decreased or loss of hearing
• Severe pain in the center of the abdomen (stomach) which may be bloated, with fever, feeling or being unwell (pancreatitis, inflammation of the pancreas)
• Pain in the kidney area and/or bladder caused by kidney stones or crystals that may move into the bladder or other parts of the urinary system. May have severe back pain and lower abdominal pain and scarcity or absence of urine. Rare (may affect up to 1 in 1,000 people):
• Decreased amount of urine, confusion, muscle cramps, irregular heartbeat (renal tubular acidosis)
• Feeling of irritability and excitability with changes in mood and abnormal behaviors
• Glaucoma, which is a blockage of fluid in the eyes that causes increased eye pressure, pain and decreased vision, blindness in one eye, temporary blindness
• Yellowing of the skin and eyes (liver inflammation, liver damage)
• Severe skin reactions, including Stevens-Johnson Syndrome, a life-threatening skin disease in which the upper layer of skin separates from the lower layer, which may present with sores in multiple sites

mucosal (such as the mouth, nose and eyes) and erythema multiforme, a condition characterized by raised red spots that can lead to the formation of blisters.

• Difficulty thinking, remembering information, or solving problems, being less attentive or aware, feeling very sleepy with little energy - these symptoms may be a sign of a high level of ammonia in the blood (hyperammonemia), which can lead to a change in brain function (hyperammonemic encephalopathy). Frequency not known (the frequency cannot be estimated based on available data):
• Maculopathy, a disease of the macula, the small part of the retina in which vision is sharpest. You may notice a change or decrease in vision.
• Toxic epidermal necrolysis, a life-threatening condition, related to Stevens-Johnson Syndrome, but even more severe, characterized by widespread blisters and shedding of the outer layers of the skin (see rare side effects). Other possible side effectsVery common (may affect more than 1 in 10 people):
• Weight loss
• Tingling, pain and/or numbness in various parts of the body
• Drowsiness, sleepiness or fatigue
• Dizziness
• Diarrhea
• Feeling unwell (nausea)
• Stuffy nose, runny nose and sore throat. Common (may affect up to 1 in 10 people):
• Weight gain
• Decreased or loss of appetite
• Feeling tired, difficulty breathing with paleness - this may be caused by a decrease in the number of red blood cells
• Difficulty falling asleep or staying asleep
• Problems with speech or speech disorders, slurred speech
• Clumsiness or lack of coordination, feeling unstable when walking
• Decreased ability to complete routine activities
• Tremor or involuntary shaking of the arms, hands or legs
• Decreased sense of touch or sensation
• Involuntary eye movements
• Altered taste
• Vision disorders, such as blurred vision, double vision, difficulty focusing
• Feeling of spinning, ringing in the ears, ear pain
• Shortness of breath
• Nosebleeds
• Feeling unwell (vomiting)
• Constipation
• Gastritis, stomach pain or discomfort
• Indigestion
• Dry mouth
• Tingling or numbness in the mouth
• Hair loss
• Joint pain or swelling
• Muscle spasms, muscle contractions, muscle weakness or muscle pain
• Chest pain (caused by muscles and bones in the chest)
• Fever
• Loss of strength
• General feeling of being unwell.
• Cough. Uncommon (may affect up to 1 in 100 people):
• Abnormal blood count, including reduced platelets (may show a tendency to bleed or bruise more easily than usual) or increased eosinophils
• Increased liver enzymes
• Elevated mood
• Not showing and feeling any emotion, abnormal suspiciousness, panic attack
• Irregular or slowed heartbeat
• Swelling of the glands of the neck, armpits or groin
• Problems with verbal communication
• Loss of saliva
• Restlessness or increased mental and physical activity
• Decreased wakefulness or alertness
• Unusual sensation or feeling that can precede a migraine or a certain type of seizure
• Loss of consciousness
• Fainting and feeling faint
• Excessive sleepiness
• Slowed or decreased movements, involuntary repetitive muscle contractions
• Poor sleep quality or disturbed sleep (e.g. waking up early in the morning)
• Distorted or impaired sense of smell
• Problems with writing or speaking, such as stuttering or constantly repeating words
• Feeling of movement under the skin
• Eye problems, including dry eyes, sensitivity to light, involuntary contractions, tearing, dilated pupils, seeing flashes of light, abnormal sensations in the eyes
• Hoarseness
• Flatulence or intestinal gas
• Heartburn
• Loss of sensitivity to touch or alteration of the sense of touch that may appear unpleasant and may affect the mouth or face
• Bleeding gums
• Feeling of fullness or bloating
• Feeling of pain or burning in the mouth
• Bad breath
• Loss of taste
• Loss of urine and/or stool, blood in the urine
• Urgent need to urinate
• Changes in skin color
• Localized swelling of the skin
• Swelling of the face
• Swelling of the joints
• Musculoskeletal stiffness or muscle fatigue
• Low levels of potassium in the blood
• Increased appetite
• Increased thirst and consumption of excessively large amounts of fluids
• Low blood pressure or decreased blood pressure when getting up which may make you feel dizzy or faint
• Shortness of breath, especially after physical activity
• Hot flashes, feeling of warmth
• Flu-like illness
• Cold extremities (e.g. hands and face)
• Feeling drunk
• Learning difficulties
• Sexual dysfunction (erectile dysfunction, loss of libido)
• Hallucinations
• Decreased verbal communication
• Feeling sad or crying spells. Rare (may affect up to 1 person in 1,000 people):
• Feeling desperate
• Involuntary tremor that may affect the tongue, hands or head while performing activities
• Diminished, unresponsive body movements
• Excessive skin sensitivity
• Impairment of the sense of smell
• Odour
• Swelling in the tissues around the eye, problems with vision in low light, lazy eye
• Raynaud's syndrome. A disorder that affects the blood vessels in the fingers of the hands and feet, in the ears, and causes pain and sensitivity to cold.
• Decreased bicarbonate in the blood that is detected by blood tests. Frequency not known (the frequency cannot be estimated based on available data):
• Swelling of the transparent membrane that covers the surface of the eye (conjunctiva)
• inflammation of the eye (uveitis) with symptoms such as redness, pain, sensitivity to light, tearing, blurred or disturbed vision due to the appearance of small spots in front of the eyes. Additional side effects in children and adolescentsSide effects in children are generally similar to those observed in adults. However, some side effects are observed more frequently in children and/or may be more severe in children than in adults. Side effects that may be more severe are: decreased or loss of sweating and increased level of acid in the blood. Side effects that may occur more frequently in children include upper respiratory tract diseases. Reporting of side effectsIf you get any side effects, talk to your doctor or pharmacist. This includes any possible side effects not listed in this leaflet. You can also report side effects directly through the national reporting system at https://www.aifa.gov.it/content/segnalazioni-reazioni-avverse. By reporting side effects you can help provide more information on the safety of this medicine.

5. How to store Topiramato Mylan Generics

Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the blister and the outer packaging after Exp. The date
of expiry refers to the last day of that month.
Store in the original packaging to protect the medicine from light and moisture.
Do not use this medicine if you notice a change in the colour of the tablets.
Do not dispose of any medicine in wastewater or household waste. Ask your pharmacist how to dispose of the
medicines you no longer use. This will help protect the environment.

6. Contents of the pack and other information

What Topiramato Mylan Generics contains
The active ingredient is topiramate.
Each film-coated tablet contains 25 mg, 50 mg, 100 mg or 200 mg of topiramate.
The other ingredients are:

Tablet core: microcrystalline cellulose, povidone K29-32, anhydrous colloidal silica, sodium starch
glycolate (type A), magnesium stearate.

Film coating: titanium dioxide (E171), hypromellose (E464), macrogol 400, polysorbate 80 (E433) (only
in 25 mg tablets), yellow iron oxide (E172) (only in 50 mg and 100 mg tablets), allura red
(E129), sunset yellow (E110) (see paragraph 2 “Topiramato 200 mg contains aluminium lake sunset yellow (E110) and
aluminium lake allura red (E129)”) and indigo carmine (E132) (only in 200 mg tablets).

Description of the appearance of Topiramato Mylan Generics and contents of the pack
Your medicine is in the form of film-coated tablets that appear as follows:
25 mg: white, round, film-coated tablets, curved outwards on both sides, with “G” engraved on
one side and “TO” above “25” on the other.
50 mg: yellow, round, film-coated tablets, curved outwards on both sides, with “G” engraved on
one side and “TO” above “50” on the other.
100 mg: yellow, round, film-coated tablets, curved outwards on both sides, with “G” engraved on
one side and “TO” above “100” on the other.
200 mg: red, round, film-coated tablets, curved outwards on both sides, with “G” engraved on
one side and “TO” above “200” on the other.
The medicine is available in aluminium blisters in packs of 10, 15, 20, 28, 30, 50, 60, 90, 100 and 200
film-coated tablets.
Not all pack sizes may be marketed.
