Read this leaflet carefully before taking this medicine as it contains
important information for you.

- This medicine has been prescribed for you only. Do not pass it on to others, even if their symptoms are the same as yours, as it may be harmful.
- If you get any side effects, even those not listed in this leaflet, tell your doctor or pharmacist. See section 4.

1. What is Dirahist and what is it used for
Dirahist is a medicine with antihistamine-cortisone activity, meaning it contains two active ingredients:
- triamcinolone, which belongs to a group of medicines called “glucocorticoids”, which work by reducing and preventing inflammation
- chlorpheniramine, an antihistamine, a substance capable of fighting allergies.
Dirahist is indicated:
- in all allergic diseases, especially if accompanied by excessive general or local release of histamine (a substance found in the body):
- urticaria (skin redness accompanied by itching)
- allergic rhinitis (hay fever)
- vasomotor rhinitis (rhinitis not of an allergic nature)
- angioedema (swelling of hands, feet, ankles, face, lips, tongue and/or throat)
- dermatitis and allergic dermatoses (inflammatory skin reactions)
- pruritus (itching)
- bronchial asthma (respiratory disease characterized by obstruction of bronchi and lungs)
- allergic purpura (appearance of spots on the skin)
- conjunctivitis and other eye conditions (eye disease characterized by intense itching and tearing)
- migraine (headache)
- headache of allergic origin
- in food allergies
- in allergic reactions following:
- transfusions
- administration of antibiotics or other medicines,
- chemical substances (such as animal venoms)
See your doctor if you do not feel better or if you feel worse.
2. What you need to know before taking Dirahist
Do not take Dirahist
- if you are allergic to triamcinolone, or to chlorpheniramine or to any of the other ingredients of this medicine (listed in section 6)
- if you suffer from tuberculosis (an infectious disease that attacks the lungs)
- if you suffer from herpes simplex (a viral disease)
- if you suffer from systemic fungal infections (infections caused by fungi that affect the whole body) The doctor will carefully assess the benefits for you against the risks:
- if you suffer from an active peptic ulcer (a lesion affecting the mucous membrane of the stomach and intestines)
- if you suffer from acute glomerulonephritis (an inflammatory disease affecting the kidneys)
- if you suffer from osteoporosis (a disease characterized by a reduction in bone mass, leading to increased bone fragility and exposure to the risk of fractures)
- if you suffer from thrombophlebitis (inflammation of veins with resulting thrombosis)
- if you have recently undergone surgery on the stomach or intestines
- if you suffer from diverticulitis (a disease of the intestine caused by inflammation of one or more diverticula, focal dilations of one or more sections of the intestine)
- if you suffer from mental disorders (thought and/or mood disorders)
- if you suffer from a local or systemic infection including mycoses (fungal infections) and exanthematous diseases (i.e. diseases that lead to the appearance of skin rashes such as measles and chickenpox).
Warnings and precautions
Consult your doctor or pharmacist before taking Dirahist.
In particular, inform your doctor if:
- you are hypothyroid (have reduced function of a gland located at the base of the neck called the thyroid) or if you suffer from hepatic cirrhosis (liver disease) as in these cases the response to glucocorticoids (medicines such as triamcinolone) may increase
- you are stressed, as in this case your doctor may decide to adjust the doses based on the extent of your stress
- you suffer from acute or chronic infections, as your doctor will need to ensure that adequate treatment with antibiotic and/or chemotherapeutic medicines is put in place
- you are emotionally unstable or have psychotic tendencies (i.e. severe alterations in an individual's mental balance, with impairment of reality testing and frequent disturbances of thought), as these symptoms may be aggravated by glucocorticoids
- an infection develops during treatment. In this case, your doctor will assess the appropriateness of initiating adequate antibiotic therapy
- a disease of the glands near the kidneys (secondary adrenal insufficiency) develops during treatment. In this case, your doctor will gradually reduce the dosage and, if deemed necessary, may prescribe you additional hormonal treatment.
- you need to be vaccinated against smallpox or undergo other vaccinations that involve activation of the immune system.
- you experience blurred vision or other visual disturbances.
During prolonged therapy and with high doses, if an alteration in water and electrolyte balance were to occur, your doctor will advise you to adjust your sodium and potassium intake to restore the balance. All glucocorticoids increase the elimination of calcium (see section 4 “Possible side effects”).
For athletes
The use of the drug without therapeutic need constitutes doping and may still result in a positive doping test.
Other medicines and Dirahist
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.
In particular, inform your doctor if you are taking Dirahist and intend to take:
- anti-inflammatory medicines belonging to the salicylate category, in this case, if you also suffer from hypoprothrombinemia (a blood disease characterized by a tendency to bleeding), the doctor will monitor you
- the smallpox vaccine (an infectious disease), this vaccine cannot be given during treatment with glucocorticoids (present in Dirahist)
- other medicines that act on the body's defense system (immune system) especially at high doses
- oral contraceptives (the pill), ketoconazole (antifungal medicine) and trioleandomycin (antibiotic), which reduce the metabolism of glucocorticoids, increasing their side effects
- barbiturates (medicines that act on the central nervous system), phenytoin (antiepileptic), carbamazepine (antiepileptic, rifampicin (antibiotic), aminoglutethimide (anti-steroid), because these medicines can reduce the effectiveness of triamcinolone
- hypnotic medicines (that induce sleep) because they can increase the depressive effect of chlorpheniramine (one of the active ingredients of Dirahist) on the central nervous system.
Dirahist with alcohol
The use of alcohol can increase the depressive effect of chlorpheniramine on the central nervous system.
Dirahist contains lactose
If your doctor has diagnosed you with intolerance to certain sugars, contact him before taking this medicine.
Pregnancy and breastfeeding
If you are pregnant, suspect you are pregnant or are planning a pregnancy, or if you are breastfeeding, ask your doctor or pharmacist for advice before taking this medicine.
If you are pregnant, the product should only be administered in cases of actual necessity under the direct supervision of the doctor.
Driving and operating machinery
As the product can cause drowsiness, you must be careful if you are driving any type of vehicle or operating machinery that requires alertness.
3. How to take Dirahist
Take this medicine following exactly the instructions of your doctor or pharmacist. If you
have any doubts, consult your doctor or pharmacist.
The recommended dose is 1 - 2 capsules 4 times a day.
The doctor will assess the optimal dose for you to control the symptoms of your allergic reaction;
if the doctor decides to reduce the dose, they must do so gradually.
If you take more Dirahist than you should
If you accidentally take a high quantity of medicine, contact your doctor or
reach the nearest hospital.
If you forget to take Dirahist
Do not take a double dose to make up for a missed dose.
Resume treatment as soon as possible.
If you stop taking Dirahist
In prolonged treatment, avoid abruptly stopping therapy.
Before stopping treatment, consult your doctor.
If you have any doubts about the use of this medicine, ask your doctor or pharmacist.
4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody gets them.
During treatment with Dirahist, the following side effects have been observed and reported, which
cannot be classified by frequency:
Effects relating to the mind
- euphoria
- insomnia
- drowsiness
- fatigue
- nervousness
- tremors
- changes in mood or personality
- severe depression
- symptoms of actual psychosis (severe alteration of an individual's mental balance, with impairment of reality testing and frequent presence of thought disorders).
Effects relating to the blood
- alterations in water and salt balance in the body (electrolyte imbalance); especially decreased potassium in the blood (hypokalemia), which rarely and in particularly predisposed patients, can lead to increased blood pressure and congestive heart failure (heart disease accompanied by difficulty breathing)
- alteration of the number of blood cells (blood dyscrasia)
Effects relating to the nervous system
- dizziness
- headache
- increased pressure in the brain (intracranial)
- extrapyramidal disorders (such as alterations in muscle tone and involuntary movements, controlled by the set of pathways and nervous centers that act directly or indirectly on movements, controlling instinctive reactions and adapting them to voluntary movement).
Effects relating to metabolism and glands
- altered functioning of the glands which can manifest as:
- irregularities in the menstrual cycle,
- cushingoid appearance (clinical condition characterized by an excess of hormones and the typical “full moon face” appearance)
- growth disorders in children
- interference with the functionality of the glands that produce hormones (pituitary-adrenal axis), particularly during stressful situations
- decreased tolerance to sugars and possible manifestation of latent diabetes mellitus (disease caused by alteration of blood sugar levels)
- increased need for medicines to lower blood sugar levels (hypoglycemic agents) in diabetics
Effects relating to muscles and bones
- alterations of muscles and bones such as osteoporosis, myopathies (i.e. muscle diseases), bone fragility.
Effects relating to the skin
- delays in wound healing
- thinning and fragility of the skin
- skin rashes
Effects relating to the stomach and intestines
- complications affecting the stomach and intestines that can lead to the appearance or activation of stomach lesions (peptic ulcer)
Effects relating to the eyes
- posterior subcapsular cataract (disease due to an opacity located at the level of the posterior capsule of the natural lens of the eye, the crystalline lens, and which manifests with blurred vision)
- increased intraocular pressure (increase in the amount of fluid produced inside the eye, which determines an increase in pressure)
- diplopia (double vision)
- blurred vision with unknown frequency (frequency cannot be defined based on available data)
Effects relating to the ear
- sensation of ringing in the ears (tinnitus).
Effects relating to blood vessels
- temporary reduction in blood pressure (transient hypotension).
Effects relating to breathing, the chest and the mediastinum
- hiccups with unknown frequency
Reporting of side effects
If you experience any side effects, including those not listed in this leaflet, please tell your
doctor. You can also report side effects directly through the national reporting system at
https://www.aifa.gov.it/content/segnalazioni-reazioni-avverse .
By reporting side effects you can help provide more information on the safety of
this medicine.
5. How to store Dirahist
Keep this medicine out of the sight and reach of children.
This medicine does not require any particular storage conditions. Store the product
in its original packaging.
Do not use this medicine after the expiry date which is reported on the package after Exp. The expiry
date refers to the last day of that month.
Do not dispose of any medicine in the drain or household waste. Ask your pharmacist how
to eliminate medicines that you no longer use. This will help to protect the environment.
6. Contents of the pack and other information
What Dirahist contains
- The active ingredients are: Triamcinolone and Chlorpheniramine (1 capsule contains 1 mg of triamcinolone and 2 mg of chlorpheniramine maleate)
- The other ingredients are: lactose, magnesium stearate, erythrosine (E 127), indigotine (E 132), titanium dioxide (E 171), gelatin.
