- This medicine has been prescribed only for you. Do not give it to other people, even if their symptoms are the same as yours, as it could be dangerous.

1. What is Remicade and what is it used for
Remicade contains the active substance infliximab. Infliximab is a monoclonal antibody – a type of
protein that binds to a specific target in the body called TNF (Tumor Necrosis
Factor) alpha.
Remicade belongs to a group of medicines called "TNF blockers". It is used in adults
for the treatment of the following inflammatory diseases:
- Rheumatoid arthritis
- Psoriatic arthritis
- Ankylosing spondylitis (Bechterew's disease)
- Psoriasis.
Remicade is also used in adults and children aged 6 years and older for:
- Crohn’s disease
- Ulcerative colitis.
Remicade works by binding selectively to TNF alpha and blocking its action. TNF alpha is
involved in the inflammatory processes of the body, so, by blocking it, it is possible to reduce
inflammation in the body.
Rheumatoid arthritis
Rheumatoid arthritis is an inflammatory disease of the joints. If you suffer from rheumatoid arthritis,
you will initially be treated with other medicines. If these medicines do not work
adequately, you will be treated with Remicade in combination with another medicine called methotrexate
to:
- Reduce the signs and symptoms of the disease
- Slow the progression of joint damage
- Improve physical function.
Psoriatic arthritis
Psoriatic arthritis is an inflammatory disease of the joints, usually accompanied by
psoriasis. If you suffer from psoriatic arthritis, you will first be treated with other medicines. If these medicines
do not work adequately, you will be treated with Remicade to:
- Reduce the signs and symptoms of the disease
- Slow the progression of joint damage
- Improve physical function.
Ankylosing spondylitis (Bechterew's disease)
Ankylosing spondylitis is an inflammatory disease of the spine. If you suffer from
ankylosing spondylitis, you will first be treated with other medicines. If these medicines do not
work adequately, you will be treated with Remicade to:
- Reduce the signs and symptoms of the disease
- Improve physical function.
Psoriasis
Psoriasis is an inflammatory disease of the skin. If you suffer from moderate to severe plaque psoriasis, you will first be treated with other medicines or other treatments, such as phototherapy. If
these medicines or treatments do not work adequately, you will be treated with Remicade
to reduce the signs and symptoms of the disease.
Ulcerative colitis
Ulcerative colitis is an inflammatory disease of the intestine. If you suffer from ulcerative colitis, you will be treated
first with other medicines. If these medicines do not work adequately, you will
be given Remicade to treat the disease.
Crohn’s disease
Crohn’s disease is an inflammatory disease of the intestine. If you suffer from Crohn’s disease, you will
be treated first with other medicines. If these medicines do not work adequately, you will
be treated with Remicade to:
- Treat active Crohn’s disease
- Reduce the number of abnormal openings (fistulas) between the intestine and the skin, for which other medicines or surgery have proven inadequate.
2. Cosa deve sapere prima di usare Remicade
Non deve esserle somministrato Remicade se:
- È allergico a infliximab o a uno qualsiasi degli altri componenti di questo medicinale (elencati al paragrafo 6).
- È allergico (ipersensibile) alle proteine del topo.
- Ha la tubercolosi (TB) o un’altra infezione grave come polmonite o sepsi.
- Ha un’insufficienza cardiaca che sia moderata o grave.
Non usi Remicade se una delle condizioni citate in precedenza possono riguardarla. Se non è sicuro,
ne parli con il medico, prima che le venga somministrato Remicade.
Avvertenze e precauzioni
Si rivolga al medico prima o durante il trattamento con Remicade se ha:
Già ricevuto in precedenza Remicade
- Informi il medico se è già stato sottoposto in passato a trattamento con Remicade e se sta ricominciando il trattamento con Remicade. Se ha interrotto il trattamento con Remicade per più di 16 settimane, esiste un maggior rischio di reazioni allergiche, quando ricomincia il trattamento con Remicade.
Infezioni
- Informi il medico prima che le venga somministrato Remicade se ha un’infezione, anche se di entità assai lieve.
- Informi il medico prima che le venga somministrato Remicade se ha vissuto o viaggiato in un’area dove infezioni chiamate istoplasmosi, coccidioidomicosi, o blastomicosi sono comuni. Queste infezioni sono causate da specifici tipi di funghi che possono colpire i polmoni o altre parti del corpo.
- Può essere più facilmente soggetto ad infezioni, quando trattato con Remicade. Se ha 65 anni o più, ha un rischio maggiore.
- Queste infezioni possono essere gravi e comprendono tubercolosi, infezioni causate da virus, funghi, batteri, o altri organismi presenti nell’ambiente e sepsi che possono essere pericolose per la vita. Informi subito il medico se presenta sintomi di infezione durante il trattamento con Remicade. I sintomi comprendono febbre, tosse, sintomi simili all’influenza, senso di malessere, arrossamento della pelle o pelle molto calda, ferite o problemi ai denti. Il medico può raccomandare di interrompere temporaneamente Remicade.
Tubercolosi (TB)
- È molto importante che informi il medico se ha mai avuto la tubercolosi o se è stato a stretto contatto con persone che hanno avuto o che hanno la tubercolosi.
- Il medico eseguirà degli esami per valutare se ha la tubercolosi. Sono stati segnalati alcuni casi di tubercolosi in pazienti trattati con Remicade, anche in pazienti che erano già stati trattati con medicinali per la TB. Il medico registrerà questi esami sulla scheda di promemoria per il paziente.
- Se il medico ritiene che sia a rischio di tubercolosi, è possibile che venga trattato con medicinali per la tubercolosi prima che le venga somministrato Remicade. Informi subito il medico se nota segni di tubercolosi durante il trattamento con Remicade. I segni comprendono tosse persistente, perdita di peso, sensazione di stanchezza, febbre, sudorazione notturna.
Virus dell’epatite B
- Informi il medico prima che le venga somministrato Remicade se è un portatore di epatite B o se in passato ha avuto l’epatite B.
- Informi il medico se crede di poter essere a rischio di contrarre l’epatite B.
- Il medico deve valutare se ha il virus dell’epatite B.
- Il trattamento con i bloccanti del TNF come Remicade può causare la riattivazione del virus dell’epatite B nei pazienti portatori di questo virus, che in alcuni casi può causare la morte.
Problemi al cuore
- Informi il medico se ha eventuali problemi al cuore, come insufficienza cardiaca lieve.
- Il medico terrà strettamente monitorato il suo cuore. Informi immediatamente il medico se nota nuovi segni di insufficienza cardiaca o un loro peggioramento durante il trattamento con Remicade. I segni comprendono respiro corto o gonfiore dei piedi.
Tumore e linfoma
- Informi il medico prima che le venga somministrato Remicade se ha o ha avuto un linfoma (un tipo di tumore del sangue) o altri tipi di tumore.
- I pazienti con artrite reumatoide grave, che hanno sofferto di questa malattia per molto tempo, possono presentare un rischio di sviluppo di linfoma maggiore.
- Bambini e adulti che assumono Remicade possono avere un aumentato rischio di sviluppare un linfoma o un altro tipo di tumore.
- Alcuni pazienti che sono stati trattati con bloccanti il TNF, compreso Remicade hanno sviluppato un raro tipo di tumore chiamato linfoma epatosplenico a cellule T. La maggior parte di questi pazienti erano adolescenti o giovani di sesso maschile e la maggior parte erano affetti da malattia di Crohn o colite ulcerosa. Questo tipo di tumore è in genere letale. Quasi tutti i
pazienti sono stati trattati anche con medicinali contenenti azatioprina o 6-mercaptopurina in
aggiunta a bloccanti del TNF.
- Alcuni pazienti trattati con infliximab hanno sviluppato alcuni tipi di cancro della pelle. Se si verificano variazioni della pelle o escrescenze sulla pelle durante o dopo la terapia, informi il medico.
- Alcune donne in trattamento per l’artrite reumatoide con Remicade hanno sviluppato un tumore della cervice uterina. Alle donne che assumono Remicade comprese quelle di oltre 60 anni di età, il medico può raccomandare uno screening periodico per il cancro cervicale.
Malattia polmonare o assidui fumatori
- Informi il medico prima che le venga somministrato Remicade se ha una malattia dei polmoni chiamata malattia polmonare cronico-ostruttiva (COPD) o se è un assiduo fumatore.
- I pazienti con COPD e che sono assidui fumatori possono presentare maggior rischio di tumore se in trattamento con Remicade.
Malattia del sistema nervoso
- Informi il medico se ha o ha avuto un problema a livello del sistema nervoso, prima che le venga somministrato Remicade. Questo comprende sclerosi multipla, sindrome di Guillain-Barré, attacchi o diagnosi di “neurite ottica”. Informi subito il medico se nota sintomi di una malattia nervosa durante il trattamento con Remicade. I segni comprendono alterazioni della visione, debolezza a braccia e gambe, torpore o formicolio in qualsiasi parte del corpo.
Aperture anomale della pelle
- Informi il medico se presenta eventuali aperture anomale della pelle (fistole), prima che le venga somministrato Remicade.
Vaccinazioni
- Informi il medico se è stato recentemente vaccinato o se ha in programma di essere vaccinato.
- Deve ricevere le vaccinazioni raccomandate prima di iniziare il trattamento con Remicade. Può ricevere alcuni vaccini durante il trattamento con Remicade, ma non deve ricevere vaccini vivi (vaccini che contengono un agente infettivo vivo, ma attenuato) mentre è in trattamento con Remicade perché possono causare infezioni.
- Se ha ricevuto Remicade mentre era in gravidanza, il bambino può avere un rischio maggiore di contrarre un’infezione in seguito alla somministrazione di un vaccino vivo durante il primo anno di vita. È importante che informi il pediatra e gli altri operatori sanitari dell’uso di Remicade affinché possano decidere quando il suo bambino debba ricevere qualsiasi vaccino, compresi i vaccini vivi come il vaccino BCG (usato per prevenire la tubercolosi).
- Se sta allattando, è importante che informi il pediatra e gli altri operatori sanitari dell’uso di Remicade prima che al suo bambino venga somministrato qualsiasi tipo di vaccino. Per ulteriori informazioni vedere il paragrafo Gravidanza e allattamento.
Agenti terapeutici infettivi
- Si rivolga al medico se ha recentemente assunto o ha in programma di assumere un trattamento con un agente terapeutico infettivo (come l’instillazione di BCG usata per il trattamento del cancro).
Operazioni o procedure odontoiatriche
- Informi il medico se dovrà essere sottoposto a eventuali interventi o cure odontoiatriche.
- Informi il chirurgo o il dentista che è in trattamento con Remicade, mostrando la scheda di promemoria per il paziente.
Problemi al fegato
- Alcuni pazienti in trattamento con Remicade hanno sviluppato gravi problemi al fegato. Informi immediatamente il medico se presenta sintomi di problemi al fegato durante il trattamento con Remicade. I segni includono ingiallimento della pelle e degli occhi, urine di
colore marrone scuro, dolore o gonfiore nella parte superiore destra dello stomaco, dolore
articolare, eruzioni cutanee o febbre.
Riduzione del numero delle cellule del sangue
- In alcuni pazienti in trattamento con Remicade, l’organismo può non non essere in grado di produrre sufficienti cellule del sangue che aiutano a combattere le infezioni o aiutano a smettere di sanguinare. Informi immediatamente il medico se presenta sintomi di riduzione del numero delle cellule del sangue durante il trattamento con Remicade. I segni includono febbre persistente, più facile sanguinamento o formazione di lividi, piccole macchie rosse o viola causate dal sanguinamento sotto la pelle o aspetto pallido.
Disturbo del sistema immunitario
- Alcuni pazienti in trattamento con Remicade hanno sviluppato sintomi di un disturbo del sistema immunitario chiamato lupus. Informi immediatamente il medico se sviluppa sintomi di lupus durante il trattamento con Remicade. I segni includono dolore articolare o un’eruzione cutanea, sulle guance o sulle braccia, che è sensibile al sole.
Bambini e adolescenti
Le informazioni sopra riportate si applicano anche a bambini e adolescenti. Inoltre:
- Alcuni pazienti bambini e adolescenti che hanno assunto medicinali bloccanti il TNF, come Remicade, hanno sviluppato tumori, compresi quelli di tipo inusuale, che talvolta hanno avuto esito fatale.
- Rispetto agli adulti, più bambini che prendevano Remicade hanno sviluppato infezioni.
- I bambini devono ricevere le vaccinazioni raccomandate prima di iniziare il trattamento con Remicade. I bambini possono ricevere alcuni vaccini durante il trattamento con Remicade, ma non devono ricevere vaccini vivi mentre sono in trattamento con Remicade.
Se non è sicuro che una delle condizioni citate in precedenza possa riguardarla, contatti il medico
prima che le somministrino Remicade.
Altri medicinali e Remicade
I pazienti con malattie infiammatorie assumono già medicinali per il trattamento della malattia. Questi
medicinali possono causare effetti indesiderati. Il medico le consiglierà quali altri medicinali deve
continuare ad assumere mentre è in terapia con Remicade.
Informi il medico se sta assumendo o ha recentemente assunto qualsiasi altro medicinale, compreso
qualsiasi altro medicinale per trattare la malattia di Crohn, la colite ulcerosa, l’artrite reumatoide, la
spondilite anchilosante, l’artrite psoriasica o la psoriasi o medicinali che si ottengono senza
prescrizione medica, come vitamine e medicinali d’erboristeria.
In particolare, informi il medico se sta usando uno qualsiasi di questi medicinali:
- Medicinali che agiscono sul sistema immunitario.
- Kineret (anakinra). Remicade e Kineret non devono essere somministrati insieme.
- Orencia (abatacept). Remicade e Orencia non devono essere somministrati insieme.
Durante il trattamento con Remicade non le devono essere somministrati vaccini vivi. Nel caso stesse
usando Remicade durante la gravidanza o se sta ricevendo Remicade durante l’allattamento, informi il
pediatra e gli altri operatori sanitari che si prendono cura del bambino dell’uso di Remicade prima che
al bambino venga somministrato qualsiasi tipo di vaccino.
Se non è sicuro che una delle condizioni citate in precedenza possa riguardarla, contatti il medico
prima che le somministrino Remicade.
Gravidanza, allattamento e fertilità
- Se è in corso una gravidanza, se sospetta o sta pianificando una gravidanza o se sta allattando con latte materno chieda consiglio al medico prima di prendere questo medicinale. Remicade deve essere utilizzato durante la gravidanza o durante l’allattamento solo se il medico lo ritiene necessario.
- Deve evitare la gravidanza quando è in trattamento con Remicade e almeno nei 6 mesi successivi all’interruzione del trattamento. Parli con il medico per l’utilizzo di un adeguato contraccettivo in questo periodo.
- Se ha ricevuto Remicade durante la gravidanza, il bambino può avere un rischio maggiore di contrarre un’infezione.
- È importante che informi il pediatra e gli altri operatori sanitari dell’uso di Remicade prima che al bambino venga somministrato qualsiasi tipo di vaccino. Se ha ricevuto Remicade durante la gravidanza, la somministrazione del vaccino BCG (usato per prevenire la tubercolosi) al bambino nei 12 mesi successivi alla nascita può dare luogo a un’infezione con gravi complicazioni, compresa la morte. I vaccini vivi come il vaccino BCG non devono essere somministrati al bambino nei 12 mesi successivi alla nascita, a meno che il pediatra raccomandi diversamente. - Se sta allattando, è importante che informi il pediatra e gli altri operatori sanitari dell’uso di Remicade prima che al bambino venga somministrato qualsiasi tipo di vaccino. I vaccini vivi non devono essere somministrati al bambino durante l’allattamento, a meno che il pediatra raccomandi diversamente.
- Nei lattanti nati da donne trattate con Remicade durante la gravidanza sono state segnalate severe diminuzioni del numero dei globuli bianchi. Se il bambino ha febbri o infezioni continue contatti immediatamente il medico del bambino.
Guida di veicoli e utilizzo di macchinari
È improbabile che Remicade abbia effetti sulla capacità di guidare o usare macchinari. Se si sente
stanco, se le gira la testa o non si sente bene dopo il trattamento con Remicade, non deve guidare o
utilizzare strumenti o macchinari.
Remicade contiene sodio
Remicade contiene meno di 1 mmol (23 mg) di sodio per dose, cioè essenzialmente “senza sodio”.
Tuttavia, prima della somministrazione, Remicade viene miscelato con una soluzione contenente
sodio. Si rivolga al medico se sta seguendo una dieta a basso contenuto di sale.
3. How to use Remicade
Rheumatoid arthritis
The usual dose is 3 mg per each kg of body weight.
Psoriatic arthritis, ankylosing spondylitis (Bechterew's disease), psoriasis, ulcerative colitis and
Crohn's disease
The usual dose is 5 mg per each kg of body weight.
How Remicade is administered
- Remicade will be administered to you by a doctor or a nurse.
- The doctor or nurse will prepare the medicine for infusion.
- The medicine will be administered via infusion (intravenous drip) (over a period of 2 hours) into a vein, usually in the arm. After the third treatment, the doctor may decide to administer the Remicade dose over a period of 1 hour.
- You will be monitored during the administration of Remicade and for the 1-2 hours afterwards.
How much Remicade is administered
- The doctor will determine the dose and the interval between doses of Remicade. This will depend on the disease, weight and response to treatment with Remicade.
- The table below shows the frequency of administration of this medicine after the first dose.
Use in children and adolescents
Remicade should be used in children only for Crohn's disease or ulcerative colitis. These
children must be 6 years of age or older.
If you are given too much Remicade
Since this medicine is administered by a doctor or a nurse, it is unlikely that you will
receive too much. The undesirable effects following an overdose of Remicade are not known.
If you forget or miss an Remicade infusion
If you forget or miss an appointment for the administration of Remicade, make another
appointment as soon as possible.
If you have any doubts about the use of this medicine, consult your doctor.

4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everyone will experience them.
Most of these effects are mild to moderate in severity. However, some patients may experience serious side
effects and require medical treatment. Side effects can also occur after treatment with Remicade has been stopped.
Tell your doctor immediately if you notice any of the following:
- Signs of an allergic reaction, such as swelling of the face, lips, mouth or throat which can cause difficulty
swallowing or breathing, rash, hives, swelling of the hands, feet or ankles. Some of these reactions can be
severe and potentially life-threatening. An allergic reaction can occur within 2 hours of the injection or
later. Further signs of allergic-type side effects that can occur up to 12 days after the injection include
muscle pain, fever, joint or jaw pain, sore throat or headache. - Signs of a heart problem, such as discomfort or pain in the chest, arm pain, stomach pain, shortness of
breath, anxiety, feeling faint, dizziness, fainting, sweating, nausea (feeling sick), vomiting, palpitations or
fluttering in the chest, a fast or slow heartbeat, and swelling of the feet. - Signs of an infection (including tuberculosis), such as fever, feeling tired, a cough that may be persistent,
shortness of breath, flu-like symptoms, weight loss, night sweats, diarrhea, wounds, pus collection in the
intestine or around the anus (abscess), dental problems or a burning sensation when urinating. - Possible signs of cancer, include but are not limited to swollen lymph nodes, weight loss, fever, unusual skin
lumps, changes in moles or skin color, or unusual vaginal bleeding. - Signs of a lung problem, such as cough, difficulty breathing or a feeling of tightness in the chest.
- Signs of neurological problems (including eye problems),such as signs of a stroke (sudden numbness or
weakness of the face, arm or leg, especially on one side of the body; sudden confusion, difficulty speaking or
understanding; difficulty seeing with one or both eyes, difficulty walking, dizziness, loss of balance or
coordination, or a severe headache), convulsions, tingling/numbness in any part of the body, or weakness
in the arms or legs, changes in vision, such as double vision or other eye problems. - Signs of a liver problem, (including hepatitis B infection, if you have had hepatitis B in the past) such as
yellowing of the skin or eyes, dark brown urine, pain or swelling in the upper right side of the stomach, joint
pain, rash or fever. - Signs of an immune system disorder, such as joint pain or a rash on the cheeks or arms that is sensitive to
the sun (lupus) or cough, shortness of breath, fever or rash (sarcoidosis). - Signs of a decrease in the number of blood cells, such as persistent fever, easier bleeding or bruising, small
red or purple spots caused by bleeding under the skin, or paleness. - Signs of severe skin problems, such as reddish spots or circular patches often with blisters in the center on the
trunk, large areas of skin peeling and flaking (exfoliation), sores in the mouth, throat, nose, genitals or eyes,
or small pus-filled bumps that can spread over the body. These skin reactions may be accompanied by fever.
Tell your doctor immediately if you notice the appearance of any of the symptoms described above.
The following side effects have been observed with Remicade:
Very common: may affect more than 1 in 10 people
- Stomach pain, feeling sick
- Viral infections such as herpes or flu
- Upper respiratory tract infections such as sinusitis
- Headache
- Infusion-related reaction
- Pain.
Common: may affect up to 1 in 10 people
- Changes in liver function, increased liver enzymes (found by blood tests)
- Lung or chest infections, such as bronchitis or pneumonia
- Difficulty breathing or pain when breathing, chest pain
- Bleeding in the stomach or intestines, diarrhea, indigestion, heartburn, constipation
- Rash like hives, itchy rash or dry skin
- Problems with balance or feeling dizzy
- Fever, increased sweating
- Circulation problems, such as low or high blood pressure
- Bruising, flushing or redness of the skin, warmth
- Feeling tired or weak
- Bacterial infections such as widespread infection, abscess or skin infection (cellulitis)
- Fungal skin infection
- Blood problems such as anemia or low white blood cell count
- Swollen lymph nodes
- Depression, sleep disorders
- Eye problems, including red eyes and infections
- Fast heartbeat (tachycardia) or palpitations
- Joint, muscle or back pain
- Urinary tract infection
- Psoriasis, skin problems such as eczema and hair loss
- Reactions at the injection site such as pain, swelling, redness or itching
- Chills, fluid buildup under the skin causing swelling
- Numbness or tingling.
Uncommon: may affect up to 1 in 100 people
- Poor blood supply, swelling of a vein
- Blood collection outside of blood vessels (hematoma) or bruising
- Skin problems such as blistering, warts, abnormal skin color or pigmentation, or swollen or thickened skin or
red, scaly and cracked skin - Severe allergic reactions (e.g., anaphylaxis), an immune system disorder called lupus, allergic reactions to
foreign proteins - Wounds that are slow to heal
- Swelling of the liver (hepatitis) or gallbladder (cholecystitis), liver damage
- Distraction, irritability, confusion, nervousness
- Eye problems including blurred or reduced vision, swollen eyes or stye
- New or worsening heart failure, slowed heart rate
- Fainting
- Convulsions, nervous system disorders
- Perforation of the intestine or intestinal blockage, stomach or abdominal cramps
- Swelling of the pancreas (pancreatitis)
- Fungal infections such as yeast infection or fungal nail infection
- Lung problems (such as edema)
- Fluid buildup around the lungs (pleural effusion)
- Narrowing of the airways in the lungs, causing difficulty breathing
- Inflammation of the membrane lining the lungs, causing sharp chest pain that worsens when breathing (pleurisy)
- Tuberculosis
- Kidney infections
- Low platelet count, excessive number of white blood cells
- Vaginal infections
- Abnormal results from blood tests showing “antibodies” against your own body
- Changes in cholesterol and fat levels in the blood
- Weight gain (for most patients, weight gain has been minimal).
Rare: may affect up to 1 in 1,000 people
- A type of blood cancer (lymphoma)
- Poor oxygen supply to organs through the blood, circulation problems such as narrowing of a blood vessel
- Inflammation of the membrane lining the brain (meningitis)
- Infections due to a weakened immune system
- Hepatitis B infection, if you have had hepatitis B in the past
- Liver inflammation caused by an immune system problem (autoimmune hepatitis)
- Liver problem that causes yellowing of the skin or eyes (jaundice)
- Swelling or growth of abnormal tissue
- Severe allergic reaction that can cause loss of consciousness and potentially life-threatening (anaphylactic
shock) - Swelling of small blood vessels (vasculitis)
- Immune disorders that can affect the lungs, skin and lymph nodes (such as sarcoidosis)
- Collection of immune system cells as a result of an inflammatory response (granulomatous lesions)
- Lack of interest or emotions
- Severe skin problems such as toxic epidermal necrolysis, Stevens-Johnson syndrome, and acute generalized
pustulosis - Other skin problems such as erythema multiforme, lichenoid reactions (itchy reddish/purple rash and/or
whitish-grey lines on mucous membranes), blisters and peeling skin or boils (furunculosis) - Severe nervous system problems, such as transverse myelitis, multiple sclerosis-like disease, optic neuritis and
Guillain-Barré syndrome - Inflammation of the eye that can cause changes in vision, including blindness
- Fluid in the membrane lining the heart (pericardial effusion)
- Severe lung problems (such as interstitial pneumonia)
- Melanoma (a type of skin cancer)
- Cervical cancer
- Low number of blood cells, including a severe decrease in the number of white blood cells
- Small red or purple spots caused by bleeding under the skin
- Abnormal values of a blood protein called “complement factor” which is part of the immune system.

- Cancer in children and adults
- A rare blood cancer that mainly affects adolescent or young males (T-cell hepatosplenic lymphoma)
- Liver failure
- Merkel cell carcinoma (a type of skin cancer)
- Kaposi's sarcoma, a rare cancer related to human herpes virus 8 infection. Kaposi's sarcoma most commonly
manifests with purple lesions on the skin. - Worsening of a condition called dermatomyositis (presents with a rash accompanied by muscle weakness)
- Heart attack
- Stroke
- Temporary loss of vision during the infusion or within 2 hours after the infusion
- Infection caused by a live vaccine due to a weakened immune system.
Additional side effects in children and adolescents
Children who have taken Remicade for Crohn's disease have shown some differences in side effects compared to
adults who have taken Remicade for Crohn's disease.
Side effects that have occurred more frequently in children are: low red blood cell count (anemia), blood in the
stool, low overall white blood cell count (leukopenia), flushing or redness, viral infections, low number of white
blood cells that fight infection (neutropenia), bone fracture, bacterial infection and allergic reactions of the
respiratory tract.

If you get any side effects, even those not listed in this leaflet, tell your doctor, pharmacist or nurse. You can
also report side effects directly through the national reporting system listed in Annex V. By reporting side
effects you can help provide more information on the safety of this medicine.
5. How to store Remicade
Remicade will generally be stored by healthcare professionals. If you should need to, the details regarding storage are as follows:
- Keep this medicine out of the sight and reach of children.
- Do not use this medicine after the expiry date which is stated on the label and the carton after “EXP”. The expiry date refers to the last day of that month.
- Store in a refrigerator (2 °C-8 °C).
- This medicine can also be stored in the original carton outside the refrigerator at a maximum of 25 °C for a single period of up to six months, but not beyond the original expiry date. In this situation, do not return it to the refrigerator. Write the new expiry date on the carton including day/month/year. Discard this medicine if not used by the new expiry date or by the expiry date printed on the carton, whichever is earlier.
- When Remicade is prepared for infusion, it is recommended to use it as soon as possible (within 3 hours). However, if the solution is prepared under completely sterile conditions, it can be stored in a refrigerator at a temperature between 2 °C and 8 °C for up to 28 days and for an additional 24 hours at 25 °C after removal from the refrigerator.
- Do not use this medicine if it is discolored or if it contains particles.
6. Contents of the pack and other information
What Remicade contains
- The active substance is infliximab. Each vial contains 100 mg of infliximab. After preparation, each mL contains 10 mg of infliximab.
- The other ingredients are sucrose, polysorbate 80, monobasic sodium phosphate and dibasic sodium phosphate.
Description of the appearance of Remicade and contents of the pack
Remicade is supplied in a glass vial containing the powder for concentrate for solution for
infusion. The powder consists of white, lyophilized granules.
Remicade is available in packs of 1, 2, 3, 4 or 5 vials. Not all pack sizes may be marketed.

Instructions for use and handling – storage conditions
Store at 2 °C-8 °C.
Remicade can be stored at temperatures not exceeding 25 °C for a single period of up to
6 months, but which does not exceed the original expiry date. The new expiry date must be
written on the packaging. After removal from the refrigerator, Remicade must not be stored
again in the refrigerator.
Instructions for use and handling – reconstitution, dilution and administration
In order to improve the traceability of biological medicines, the trade name and batch number
of the administered product must be clearly recorded.
- 1. Calculate the dose and the number of vials of Remicade needed. Each vial of Remicade contains 100 mg of infliximab. Calculate the total volume required of the reconstituted Remicade solution.
- 2. Under aseptic conditions, reconstitute each vial of Remicade with 10 mL of water for injections using a syringe with a 21 gauge (0.8 mm) or smaller needle. Remove the aluminum tab from the vial and clean the top with a cotton swab soaked in 70% alcohol. Insert the syringe needle into the vial through the center of the rubber stopper and direct the stream of water for injections towards the glass wall of the vial. Gently rotate the solution to completely dissolve the lyophilized powder. Do not shake vigorously or for a long time. DO NOT SHAKE. Foam formation may occur during reconstitution. Allow the reconstituted solution to stand for 5 minutes. Check that the solution is from colorless to yellowish and opalescent; the solution may contain some small translucent particles, as infliximab is a protein. Do not use the solution if you notice opaque particles, a color change or other foreign matter.
- 3. Dilute the total volume of the reconstituted Remicade dose to 250 mL with sodium chloride 9 mg/mL (0.9%) solution for infusion. Do not dilute the reconstituted Remicade solution with any other diluent. Dilution can be carried out by withdrawing a volume of sodium chloride 9 mg/mL (0.9%) solution for infusion, from the glass bottle or 250 mL infusion bag, equal to the volume of reconstituted Remicade. Slowly add the total volume of reconstituted Remicade solution to the 250 mL bottle or infusion bag. Mix gently. For volumes greater than 250 mL, use a larger infusion bag (e.g., 500 mL, 1000 mL) or multiple 250 mL infusion bags, in order to ensure that the concentration of the infusion solution does not exceed 4 mg/mL. If stored in the refrigerator after reconstitution and dilution, the infusion solution must be brought to room temperature at 25 °C for 3 hours before proceeding as indicated in point 4 (infusion). Storage for periods longer than 24 hours at 2 °C-8 °C applies only to the preparation of Remicade in the infusion bag.
- 4. Administer the infusion solution for an infusion time of no less than that recommended. Use only an infusion set with a sterile, non-pyrogenic, low protein-binding in-line filter (pore size 1.2 micrometers or less). Since no preservative is contained, it is recommended to start the administration of the intravenous infusion solution as soon as possible and within 3 hours of reconstitution and dilution. If not used immediately, the storage times during use and the storage conditions prior to use are the responsibility of the user and should not normally exceed 24 hours at a temperature between 2 °C and 8 °C, unless the
reconstitution/dilution has taken place under controlled and validated aseptic conditions. Unused
solution must not be stored for subsequent use.
- 5. Compatibility studies have not been conducted to evaluate the combined administration of Remicade with other agents. Do not administer Remicade concurrently with other medicines in the same intravenous line.
- 6. Before administration, visually inspect Remicade for the presence of particles or a change in color. If opaque particles, a color change or foreign particles are observed, do not use it.
- 7. Unused medicine and waste materials derived from such medicine must be disposed of in accordance with local regulations.
