Read this leaflet carefully before you start taking this medicine as it contains important information for you.

• Keep this leaflet. You may need to read it again.
• If you have any questions, ask your doctor or pharmacist.
• This medicine has been prescribed for you only. Do not pass it on to others, even if their symptoms are the same as yours, as it could be harmful.
• If you get any side effects, even those not listed in this leaflet, tell your doctor or pharmacist. See section 4.

1. What is Presstwo and what is it used for

Presstwo contains two active ingredients called ramipril and amlodipine. Ramipril belongs to a group
of medicines called ACE inhibitors (Angiotensin Converting Enzyme Inhibitors), while
amlodipine belongs to a group of medicines called calcium antagonists.
Ramipril works by:

• decreasing the production, by the body, of substances that can cause an increase in blood pressure
• relaxing and dilating blood vessels
• making it easier for the heart to pump blood around the body.

Amlodipine works by:

• relaxing and dilating blood vessels, so that blood can flow through them more easily.

Presstwo can be used to treat high blood pressure (hypertension) in adult patients,
who are adequately controlled with the individual active ingredients administered simultaneously,
at the same dose levels as the combination, but in separate tablets.

2. What you need to know before taking Presstwo

Do not take Presstwo:

• if you are allergic to ramipril, amlodipine, any other calcium antagonist or ACE inhibitor, or to any of the other ingredients of this medicine (listed in section 6). Signs of an allergic reaction can include skin rash, difficulty breathing or swallowing, swelling of the lips, face, throat or tongue.
• if you suffer from heart failure after a heart attack.
• if you have a severe narrowing of the aortic heart valve (aortic stenosis) or cardiogenic shock (a condition in which the heart is unable to provide enough blood to the body).
• if you have ever had a serious allergic reaction called “angioedema” in the past. Symptoms include itching, skin rash (urticaria), red patches on the hands, feet and throat, swelling of the throat and tongue, swelling around the eyes and lips, difficulty breathing and swallowing.
• if you are on dialysis or undergoing any other type of blood filtration. Depending on the machine used, Presstwo may not be suitable for you.
• if you have kidney problems where blood flow to the kidneys is reduced (renal artery stenosis)
• if you suffer from liver problems.
• if you have diabetes or your kidney function is impaired and are being treated with a medicine that lowers blood pressure, containing aliskiren.
• if you have taken or are currently taking sacubitril/valsartan, a medicine used to treat a type of long-term (chronic) heart failure in adults, as the risk of angioedema is increased (rapid swelling under the skin in an area such as the throat).
• during the last 6 months of pregnancy (see the section below “Pregnancy and breastfeeding”).
• if you have abnormally low or unstable blood pressure. Your doctor will need to make this assessment.

Do not take Presstwo if any of these conditions apply to you. If you are not sure, ask your doctor before
taking Presstwo.

Warnings and precautions
Talk to your doctor or pharmacist before taking Presstwo.

• if you are elderly
• if you have heart or kidney problems.
• if you are at risk of circulatory disorders to the heart or brain in case of acute hypotension
• if you have a severe increase in blood pressure.
• if you have lost a lot of salts or body fluids (due to illness (vomiting), diarrhea, excessive sweating, or following a low-salt diet, or after taking oral diuretics for a long time or after dialysis).
• if you are about to undergo treatment to reduce allergy to bee or wasp stings (desensitization).
• if you are about to undergo anesthesia that may be administered for surgery or dental work. It may be necessary to stop treatment with Presstwo the day before. Ask your doctor for advice.
• if you are taking any of the following medicines as they may increase the risk of angioedema, a serious allergic reaction:
• racecadotril, a medicine used to treat diarrhea
• medicines used to prevent organ transplant rejection and for cancer (e.g. temsirolimus, sirolimus, everolimus)
• vildagliptin, a medicine used to treat diabetes
• if you have a high amount of potassium in your blood (shown by a blood test).
• if you are taking medicines or are in conditions that may lower sodium levels in your blood. Your doctor may prescribe blood tests at regular intervals, especially to check sodium levels in your blood, particularly if you are elderly.
• if you have a connective tissue disease such as scleroderma or systemic lupus erythematosus.
• if you are dark-skinned You have a higher risk of:
• sudden, often very painful swelling of the deep layers of the skin, especially on the face (angioedema)
• reduced effect of ramipril
• if you have a cough Tell your doctor if it gets worse.
• if you are taking any of the following medicines used to treat high blood pressure:
• an angiotensin II receptor blocker (AIIRA) (also known as sartans - for example valsartan, telmisartan, irbesartan), especially if you have kidney problems related to diabetes.
• aliskiren. Your doctor may check your kidney function, blood pressure and the amount of electrolytes in your blood (e.g. potassium) at regular intervals. See also the information in the section “Do not take Presstwo”.
• you must tell your doctor if you think you may be pregnant (or could become pregnant). Presstwo is not recommended during the first 3 months of pregnancy and can cause serious harm to the baby after the first three months of pregnancy (see section “Pregnancy and breastfeeding”).

If any of the above conditions apply to you (or you are not sure), ask your doctor before
taking Presstwo.
It is recommended that your doctor monitors the white blood cell count. More frequent monitoring is recommended:

• at the beginning of treatment
• in patients with impaired renal function or connective tissue vascular diseases or
• when medicines that affect blood cell counts are used

Children and adolescents
The use of Presstwo is not recommended in children and adolescents under 18 years of age due to
lack of safety and efficacy data for this medicine in this population

Other medicines and Presstwo
Tell your doctor or pharmacist if you are taking, have recently taken or might take
any other medicines. This is because Presstwo can affect the mechanism of action of other
medicines. Also some medicines can affect the mechanism of action of Presstwo.
Tell your doctor if you are taking any of the following medicines. They may make Presstwo
work less well:

• medicines used to relieve pain and inflammation (e.g. non-steroidal anti-inflammatory drugs [NSAIDs] such as ibuprofen, indometacin and acetylsalicylic acid)
• medicines used to treat low blood pressure, shock, heart failure, asthma or allergies such as epinephrine, norepinephrine, adrenaline, isoproterenol, dobutamine or dopamine. Your doctor will need to check your blood pressure.
• rifampicin (antibiotic used to treat tuberculosis)
• St John's wort (herbal treatment for depression)

Tell your doctor if you are taking any of the following medicines. These medicines, if taken with
Presstwo, may increase the likelihood of adverse effects:

• medicines used to relieve pain and inflammation (e.g. non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen, indometacin and acetylsalicylic acid)
• cancer medicines (chemotherapy)
• temsirolimus (against cancer)
• medicines that are more often used to prevent organ rejection (sirolimus, everolimus and other medicines belonging to the class of mTOR inhibitors). See the section “Warnings and precautions”.
• sacubitril/valsartan (used to treat heart problems) Do not take Presstwo if you have taken or are currently taking sacubitril/valsartan (see section “Do not take Presstwo”).
• diuretics (water tablets) such as furosemide
• medicines that lower blood pressure

Your doctor may need to adjust the dose and/or take other precautions:
If you are taking an angiotensin II receptor blocker (ARB) or aliskiren (see
also what is stated under “Do not take Presstwo” and “Warnings and precautions”).

• erythromycin, clarithromycin (antibiotics)
• potassium supplements (including salt substitutes), potassium-sparing diuretics (a certain group of diuretics, e.g. spironolactone, triamterene, amiloride) and other medicines that can increase the amount of potassium in the blood (e.g. trimethoprim and co-trimoxazole, also known as trimethoprim/sulfamethoxazole for infections caused by bacteria; cyclosporine and tacrolimus, immunosuppressant medicines used to prevent organ transplant rejection and heparin, a medicine used to thin the blood and prevent blood clots) Careful monitoring of potassium in the blood is necessary when using these medicines together.
• steroid medicines for inflammation such as prednisolone
• allopurinol (used to lower uric acid levels in the blood)
• procainamide (for heart rhythm problems)
• medicines that can change the number of blood cells
• ketoconazole, itraconazole (used to treat fungal infections)
• ritonavir, indinavir, nelfinavir (used to treat patients infected with HIV)
• dantrolene (infusion for severe body temperature abnormalities)
• vildagliptin (used to treat diabetes)
• racecadotril (used against diarrhea)
• verapamil, diltiazem (for the treatment of some heart and high blood pressure diseases)

Tell your doctor if you are taking any of the following medicines. The mechanism of action of these
medicines may be affected by Presstwo:

• medicines for diabetes such as oral hypoglycemic medicines and insulin Presstwo may lower blood sugar levels. Carefully monitor your blood sugar levels while taking Presstwo.
• lithium (for mental health problems) Presstwo may increase the amount of lithium in your blood. The amount of lithium will need to be carefully monitored by your doctor.
• simvastatin (medicine to lower cholesterol) Amlodipine increases exposure to simvastatin. Your doctor should reduce the dose of simvastatin if you take Presstwo.

If any of the above conditions apply to you (or you are not sure), ask your doctor before
taking Presstwo.

Presstwo with food, drinks and alcohol

• Grapefruit juice or grapefruit should not be consumed by people who are taking Presstwo. This is because grapefruit or grapefruit juice can lead to an increase in the levels of the active ingredient amlodipine in the blood, which can increase the blood pressure lowering effect of Presstwo.
• Drinking alcohol during treatment with Presstwo may make you feel dizzy or lightheaded. If you are concerned about how much you can drink while taking Presstwo, talk to your doctor as medicines used to lower blood pressure and alcohol can intensify their respective sedative effects.

Pregnancy and breastfeeding
If you are pregnant, suspect you may be pregnant or are planning to become pregnant, ask your doctor or
pharmacist for advice before taking this medicine.
You should not take Presstwo during the first 12 weeks of pregnancy and should not take
Presstwo after the 13th week as use in pregnancy may be harmful to the baby.
If you become pregnant during treatment with Presstwo, tell your doctor immediately. Switching to a
suitable alternative treatment should be undertaken before a planned pregnancy.
You should not take Presstwo if you are breastfeeding.

Driving and operating machinery
Presstwo may affect your ability to drive or operate machinery. If the medicine makes you
feel unwell, dizzy, tired or headache, do not drive or operate machinery. This may happen
especially at the beginning of treatment with Presstwo or when the dose is increased

3. How to take Presstwo

Take this medicine following exactly the instructions of your doctor or pharmacist. If you
have any doubts, ask your doctor or pharmacist.

Taking this medicine

• Take this medicine by mouth at the same time of each day, during or away from meals
• Swallow the hard capsule whole with some liquid.
• Do not crush or chew the hard capsule.
• Do not take this medicine with grapefruit juice

How much to take

• The recommended dose is 1 capsule a day of the dosage established by your doctor.
• Depending on the effect, your doctor may modify the dose.
• The maximum dose is 1 capsule once a day, of a dosage of 10 mg/10 mg.

Kidney disease
The doctor may modify the dosage if you have kidney problems.

Liver disease
Do not take this medicine if you have liver problems (see section “Do not take Presstwo”)

Elderly
Your doctor may reduce the initial dose of Presstwo and will adjust your treatment more slowly.
In very elderly and debilitated patients, the administration of Presstwo is not recommended.

Use in children and adolescents
The use of Presstwo is not recommended in children and adolescents under 18 years of age due to
the lack of safety and efficacy data.
If you feel that the effect of the medicine is too strong or too weak, consult your doctor.

If you take more Presstwo than you should
Taking too many capsules can cause a lowering, even dangerous, of blood pressure.
You may experience dizziness, lightheadedness, fainting or feel weak. If the drop in blood pressure is particularly
severe, shock may occur. The skin may become cold and clammy and you may lose
consciousness. Inform your doctor immediately or go to the nearest hospital immediately.
Do not drive to the hospital, but have someone accompany you or call an ambulance. Bring with you the
package, so that the doctor knows what you have taken.
Excess fluid may accumulate in the lungs (pulmonary edema) causing shortness of breath
which can develop up to 24-48 hours after taking it.

If you forget to take Presstwo
If you forget to take a capsule, skip that dose completely. Take the next dose
at the usual time. Do not take a double dose to make up for the missed dose.

If you stop taking Presstwo
Do not suddenly stop taking the medicine or change the prescribed dose before
consulting your doctor, as in these cases the disease may temporarily worsen.
If you have any doubts about the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them.

Stop taking Presstwo and contact your doctor immediatelyif you notice any of the following serious side effects, as you may need urgent medical treatment:

• Swelling of the face, lips or throat making it difficult to swallow or breathe, as well as itching or rash. This can be a sign of a severe allergic reaction to this medicine.
• Severe skin reactions including rash, mouth sores, blistering rash of the lips, eyes and mouth, worsening of a pre-existing skin condition, redness, severe itching, blistering rash, peeling and blistering of the skin, inflammation of the mucous membranes (such as Stevens-Johnson syndrome, toxic epidermal necrolysis or erythema multiforme) or other allergic reactions.

Tell your doctor immediately if you experience:

• Increased heart rate, irregular or forceful heartbeat (palpitations), slow or irregular heartbeat, chest pain, feeling of tightness in the chest (angina pectoris) or more serious problems including heart attack and stroke.
• Shortness of breath, difficulty breathing, wheezing or cough. These could be signs of lung problems
• Easier bruising, bleeding that lasts longer than normal, any sign of bleeding (e.g. bleeding gums), purple spots on the skin or easy onset of infections, sore throat and fever, feeling tired, fainting, dizziness or pale skin. These can be signs of problems with blood circulation or the bone marrow.
• Severe stomach pain that may extend to your back. This can be a sign of inflammation of the pancreas (pancreatitis).
• Fever, chills, fatigue, loss of appetite, stomach pain, feeling unwell, dark urine, yellowing of the skin or eyes (jaundice). These can be signs of liver problems such as inflammation of the liver (hepatitis) or liver damage.
• noticeable reduction in urine production, swelling, loss of appetite, nausea, vomiting, fatigue, difficulty breathing, abnormal heart rhythms. These can be signs of serious kidney problems.

Other side effects
Tell your doctor if you experience any of the following side effects

Very common:may affect more than 1 in 10 people

• Edema

Common:may affect up to 1 in 10 people

• Drowsiness (especially at the beginning of treatment).
• Palpitations (feeling of a strong heartbeat), hot flushes.
• Swelling of the ankles.
• Headache or feeling tired.
• Feeling dizzy. This can happen especially when you start therapy with Presstwo.
• Fainting (syncope), hypotension (abnormally low blood pressure), especially when getting up or sitting up too quickly.
• Vision disturbances (including double vision)
• Dry, tickly cough, sinus inflammation (sinusitis) or bronchitis, shortness of breath.
• Stomach or intestinal pain, altered bowel habits (including diarrhea or constipation), abdominal discomfort, indigestion, feeling or being sick.
• Inflammation of the stomach or intestine
• Rash with or without swelling.
• Chest pain
• Weakness
• Muscle cramps or pain.
• Blood tests showing too much potassium in the blood than normal.

Uncommon: may affect up to 1 in 100 people

• Mood changes, feeling depressed, anxiety, more nervous than usual or restlessness, sleep problems (difficulty falling asleep)
• Tremors
• Vision disturbances (including blurred vision)
• Tinnitus (ringing in the ears)
• Cold/runny nose
• Cough
• Hair loss
• Itching, widespread rash, patchy purple spots on the skin (purpura), changes in skin color
• Problems with urination, greater amount of urine than usual during the day, increased number of times to urinate, increased urge to urinate especially at night
• Worsening of pre-existing proteinuria (higher than normal protein in the urine)
• Feeling unwell
• Back pain
• Weight gain or loss
• Breast enlargement in men
• Feeling lightheaded
• Itching and unusual skin sensations such as numbness, tingling, prickling, burning or chills (paresthesia)
• Loss of skin sensation
• Loss or alteration of taste
• Blocked nose, difficulty breathing or wheezing, worsening of asthma
• An intestinal swelling called “intestinal angioedema” which presents with symptoms such as abdominal pain, vomiting and diarrhea
• Inflammation of the pancreas (pancreatitis)
• Dry mouth
• Stomach pain, nausea (gastritis)
• Increased sweating compared to normal
• Loss or decrease of appetite (anorexia)
• Swollen arms and legs. This could indicate that your body is retaining more water than normal.
• Joint pain
• Fever
• Impotence in men, decreased sexual drive in men or women
• Blood tests showing an increase in the number of some white blood cells (eosinophilia)
• Blood tests showing changes in the function of liver, pancreas or kidneys
• Heart attack, heart vessel disease (myocardial ischemia), chest tightness and pain (angina pectoris), fast, irregular or slow heartbeats
• Swelling of the face, lips or throat (see warnings at the beginning of paragraph 4)
• Reduced kidney function including severe kidney disease (see warnings at the beginning of paragraph 4)
• Pain

Rare:may affect up to 1 in 1000

• Feeling confused.
• Balance problems
• Red or swollen tongue
• Severe peeling or flaking of the skin, itching, rash with swelling.
• Nail problems (e.g. loss or separation of a nail from the nail bed).
• Rash or bruising.
• Spots on the skin and coldness in the extremities.
• Red, itchy, swollen or watery eyes
• Hearing problems
• Blood tests showing a decrease in the number of red blood cells, white blood cells, platelets or in the amount of hemoglobin.
• Inflammation of blood vessels
• Narrowing of blood vessels
• yellowing of the skin (cholestatic jaundice), damaged liver cells

Very rare: (may affect up to 1 in 10,000)

• Allergic reactions
• Inflammation of the liver
• Increased sensitivity to the sun
• High blood sugar
• Increased muscle tension
• Nerve disorder that can cause muscle weakness, tingling or numbness
• Swollen gums
• Severe allergic reactions (see warnings at the beginning of paragraph 4)

Not known: (frequency cannot be estimated on the basis of available data)
Tell your doctor if any of the following effects become more severe or last longer than a few days

• Difficulty concentrating
• Inflammation of the oral mucosa with small ulcers
• Blood tests showing too little blood cells
• Blood tests showing a low amount of sodium in the blood than usual.
• Fingers and toes that change color when cold and then tingle or feel pain when warmed (Raynaud's phenomenon)
• Delayed or altered reactions
• Burning sensation
• Alteration in the perception of smells
• Thickened patches of skin that are red/silver in color (psoriasis) or a rash that looks silvery in color
• Rash on the mucous membrane (exanthem)
• Bone marrow failure
• Severe allergic reactions
• Circulation problems in the brain including stroke
• Increase of some antibodies
• Concentrated urine (dark in color), feeling unwell or sick, muscle cramps, confusion and convulsions that may be due to inappropriate secretion of ADH (antidiuretic hormone). If you are affected by these symptoms, contact your doctor as soon as possible.
• Tremor, rigid posture, expressionless face, slow and unsteady movements and gait.

If you get any side effects, even those not listed in this leaflet, tell your doctor or pharmacist. You can also report side effects directly through the national reporting system at https://www.aifa.gov.it/content/segnalazioni-reazioni-avverse.
By reporting side effects you can help provide more information on the safety of this medicine.

5. How to store Presstwo

Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the package and blister after
“Exp.”. The expiry date refers to the last day of that month.
Store at a temperature below 25 °C.
Do not dispose of any medicine in wastewater or household waste. Ask your pharmacist how to
dispose of medicines you no longer use. This will help protect the environment.

• The active ingredients are ramipril and amlodipine.

Each hard capsule contains 5 mg of ramipril and 5 mg of amlodipine (as 6.934 mg of amlodipine

Each hard capsule contains 10 mg of ramipril and 5 mg of amlodipine (as 6.934 mg of amlodipine
besilate)
