1. What is Plaquenil and what is it used for
Plaquenil contains hydroxychloroquine sulfate, which belongs to a class of medicines called
"antiprotozoal-antirheumatic”.
Plaquenil is indicated for:
- adults:
- treatment of active and chronic rheumatoid arthritis, an inflammatory disease of the joints;
- treatment of discoid and disseminated lupus erythematosus, a disease of the body's defense system characterized by raised skin rashes sensitive to the sun, located mainly on the face, arms, chest and back
- children:
- treatment of juvenile idiopathic arthritis (together with other medicines), an inflammatory disease of the joints;
- treatment of systemic and discoid lupus erythematosus, a disease of the body's defense system characterized by skin rashes sensitive to the sun, inflammation of the joints with pain and alterations of the blood and organs and systems such as the nervous system, kidneys, heart and lungs.
2. Cosa deve sapere prima di prendere Plaquenil
Non prenda Plaquenil
- se è allergico al principio attivo e ad altri medicinali simili a idrossiclorochina (4- aminochinolinici) o ad uno qualsiasi degli altri componenti di questo medicinale (elencati al
paragrafo 6)
- se ha delle alterazioni di una part of the eye called the retina and the visual field caused by medicines similar to hydroxychloroquine (4-aminoquinolines)
- se ha una malattia della retina dell’occhio chiamata maculopatia
- se il paziente è un bambino di età inferiore a 6 anni e con peso corporeo inferiore a 31 kg.
Avvertenze e precauzioni
Si rivolga al medico o al farmacista prima di prendere Plaquenil.
Deve usare Plaquenil con particolare cautela se:
- soffre o ha sofferto di miastenia (una malattia caratterizzata da debolezza muscolare generale che include, in alcuni casi, i muscoli usati per la respirazione). Può notare un peggioramento di sintomi quali debolezza muscolare, difficoltà di deglutizione, visione doppia, abbassamento della palpebra superiore, ecc…;
- soffre di una malattia al fegato (insufficienza epatica) o ai reni (insufficienza renale) (il medico può ritenere necessario ridurre la dose di Plaquenil) o sta assumendo medicinali che agiscono su questi organi;
- soffre di un’infezione cronica non attiva da virus dell’epatite B;
- ha dei disturbi allo stomaco e all’intestino (gastrointestinali), al sistema nervoso (neurologici) o al sangue (ematologici);
- è ipersensibile ad una sostanza detta “chinina”;
- le è stata diagnosticata la mancanza di una sostanza presente nel sangue denominata “glucosio- 6-fosfato deidrogenasi”, o una malattia del sangue detta porfiria che influisce sul suo metabolismo o una malattia della pelle chiamata psoriasi;
- sta prendendo medicinali che possono provocare infiammazioni della pelle (dermatiti);
- se lei soffre di prolungamento dell’intervallo QT congenito o acquisito (documentato con l’elettrocardiogramma);
- sta prendendo medicinali noti per influire sul ritmo del cuore (vedere “Altri medicinali e Plaquenil”).
Se non rileva un effettivo miglioramento dell’artrite reumatoide entro sei mesi di trattamento con
Plaquenil, contatti il medico che valuterà se sospendere la terapia.
I pazienti in trattamento con Plaquenil possono ammalarsi al cuore (cardiomiopatie), con conseguente
incapacità del cuore a pompare una quantità sufficiente di sangue all’organismo (scompenso cardiaco);
alcuni di questi casi hanno portato alla morte.
Altri disturbi del cuore dovuti all’uso prolungato del medicinale possono essere: alterazioni del battito
del cuore (blocco di branca/blocco atrio-ventricolare), o un aumento di una parte del cuore (ipertrofia
biventricolare). Il medico la sottoporrà ad un controllo clinico del cuore e potrà decidere di sospendere
il trattamento con Plaquenil.
Dopo trattamenti prolungati con dosi elevate di medicinali simili a idrossiclorochina (derivati
chinolinici) sono stati segnalati, in rari casi, disturbi nervosi. È importante pertanto che lei si attenga alla
dose che le ha prescritto il medico. In alcuni pazienti che hanno ricevuto dosi elevate e prolungate di
medicinali simili a idrossiclorochina (derivati 4-aminochinolinici) per il trattamento dell'artrite
reumatoide e del lupus eritematoso, sono state osservate lesioni permanenti alla retina dell’occhio.
Se il medico prevede che lei debba seguire una terapia prolungata con Plaquenil, inizialmente le farà
fare un esame approfondito dell’occhio (determinazione dell’acuità visiva, del campo visivo, della
visione dei colori e l’esame del fundus). Questo esame verrà ripetuto almeno una volta all’anno.
La tossicità sulla retina dell’occhio è in gran parte in rapporto alla dose assunta del medicinale. Il rischio
di danno alla retina è lieve alle dosi basse. Non superi la dose giornaliera raccomandata, perché può
aumentare decisamente il rischio di tossicità alla retina.
Il medico le farà ripetere gli esami all’occhio più frequentemente se lei:
- assume una dose giornaliera superiore a 6,5 mg/kg di peso ideale (calcolato come se lei fosse un soggetto magro)
- ha problemi ai reni (insufficienza renale);
- assume una dose totale di Plaquenil superiore a 200 g;
- è anziano;
- la sua vista è diminuita.
Se ha alterazioni della vista (alterazioni dell’acuità visiva, del campo visivo, della visione dei colori e
della retina - quali alterazioni del pigmento, perdita del riflesso foveale - o qualsiasi disturbo visivo
considerato anomalo dal medico), il suo medico le farà sospendere immediatamente il medicinale e la
terrà sotto stretta osservazione per scoprire un’eventuale progressione di queste alterazioni. Le lesioni
della retina (e i disturbi della vista) possono aggravarsi anche dopo la sospensione del trattamento con
Plaquenil (vedere “Possibili effetti indesiderati”).
Alcune persone trattate con Plaquenil possono manifestare disturbi mentali quali pensieri irrazionali,
ansia, allucinazioni, sensazione di confusione o di depressione, compresi pensieri di autolesionismo o
suicidio, anche in soggetti che non hanno mai avuto prima simili problemi. Se lei o chi la assiste nota
uno qualsiasi di questi effetti indesiderati (vedere paragrafo 4) si rivolga immediatamente al medico e
interrompa il trattamento se manifesta pensieri di autolesionismo o suicidio.
Con Plaquenil possono presentarsi dei disturbi, detti extrapiramidali, conseguenti ad alterazioni di una
parte del sistema nervoso (vedere “Possibili effetti indesiderati”).
Plaquenil provoca una grave diminuzione dei valori di zucchero nel sangue (ipoglicemia), inclusa la
perdita di coscienza che può metterla in pericolo di vita sia che lei prenda o non prenda medicinali
antidiabetici.
Se lei, durante il trattamento con idrossiclorochina presenta i disturbi dovuti ad un abbassamento degli
zuccheri nel sangue (ipoglicemia) che le sono stati spiegati dal medico, verrà sottoposto a controlli del
sangue e si potrebbe rendere necessaria una modifica della terapia.
Se lei sta seguendo una terapia a lungo termine con Plaquenil, il medico le farà eseguire periodicamente
alcuni esami del sangue (esame emocromocitometrico) e le sospenderà la somministrazione di Plaquenil
in caso di risultati anomali. Il medico la sottoporrà anche ad un esame della funzionalità dei muscoli e
controllerà la risposta muscolare ad alcuni stimoli nervosi (riflessi rotuleo ed achilleo). Qualora si
manifesti debolezza ai muscoli (astenia) le verrà sospeso il trattamento con Plaquenil.
Sono state segnalate gravi eruzioni cutanee con l’uso di idrossiclorochina (vedere paragrafo 4 Possibili
effetti indesiderati). Spesso, l’eruzione cutanea può comportare ulcere della bocca, della gola, del naso,
dei genitali e congiuntivite (occhi rossi e gonfi). Queste gravi eruzioni cutanee sono spesso precedute
da sintomi simil-influenzali, come febbre, cefalea e dolore corporeo. L’eruzione cutanea può evolvere
in eruzione cutanea con vescicole diffuse e desquamazione della pelle. Se sviluppa questi sintomi
cutanei, interrompa l’assunzione di idrossiclorochina e contatti immediatamente il medico.
Se lei dovesse notare la comparsa su tutto il corpo di un eritema accompagnato da pustole e febbre, il
medico prenderà in considerazione la diagnosi di pustolosi esantematica acuta generalizzata (AGEP).
In tale caso il trattamento con Plaquenil deve essere interrotto (vedere “Possibili effetti indesiderati”).
L’idrossiclorochina può causare disturbi cardiaci, renali o muscolari. Chieda al medico di informarla su
segni e sintomi della fosfolipidosi indotta da farmaci. Potrebbe essere necessario interrompere
l’assunzione di idrossiclorochina.
Alcuni dati ottenuti da esperimenti indicano un possibile rischio di mutazioni genetiche. Nell’uomo i
dati sono insufficienti per poter escludere un aumento del rischio di cancro nei pazienti in trattamento a
lungo termine.
Bambini e adolescenti
I bambini più piccoli sono particolarmente sensibili agli effetti tossici di Plaquenil e di medicinali simili
(4-aminochinoline); se lei ha un bambino conservi Plaquenil fuori dalla sua portata e nel caso il bambino
debba assumere il medicinale osservi scrupolosamente le indicazioni del medico.
Altri medicinali e Plaquenil
Informi il medico o il farmacista se sta assumendo, ha recentemente assunto o potrebbe assumere
qualsiasi altro medicinale, compresi i medicinali ottenuti senza prescrizione medica.
In particolare questi sono:
- Medicinali noti per influire sul ritmo del cuore. Questi comprendono medicinali usati
- per il ritmo cardiaco anormale (antiaritmici),
- per la depressione (antidepressivi triciclici),
- per i disturbi psichiatrici (antipsicotici),
- per le infezioni batteriche (per esempio moxifloxacina, macrolidi compresi azitromicina, roxitromicina, spiramicina),
- per il trattamento dell’HIV (per esempio saquinavir),
- per infezioni fungine (per esempio fluconazolo),
- per infezioni parassitarie (per esempio pentamidina),
- o contro la malaria (ad esempio alofantrina).
- Insulina o altri medicinali per il diabete
- Medicinali per la malaria come, per esempio, la meflochina (poiché aumenta il rischio di convulsioni)
- Medicinali per l’epilessia (convulsioni), in particolare fenobarbital, fenitoina, carbamazepina
- Tamoxifene, usato per il tumore del seno
- Agalsidasi (usato per una malattia rara chiamata malattia di Fabry)
- Medicinali usati per infezioni batteriche (come rifampicina, claritromicina, e quelli chiamati “aminoglicosidi” come gentamicina, neomicina o tobramicina)
- Neostigmina e piridostigmina, usati per la debolezza muscolare (miastenia grave)
- Vaccino per la rabbia
- Antiacidi (contro il bruciore di stomaco) e caolino. Prenda Plaquenil ad almeno 2 ore di distanza da
questi.
- Cimetidina (usata per i bruciori di stomaco)
- Medicinali usati per infezioni da funghi (come itraconazolo)
- Medicinali usati per disturbi lipidici (come gemfibrozil, statine)
- Medicinali usati per il trattamento dell’HIV (come ritonavir)
- Medicinali usati per i trapianti di organo o per disturbi del sistema immunitario (come la ciclosporina)
- Medicinali usati per i coaguli di sangue (come per es. dabigatran, clopidogrel)
- Medicinali usati per malattie del cuore (per es. digossina, flecainide, propafenone, chinidina e metoprololo)
- Praziquantal (usato contro i parassiti)
- Un trattamento per la depressione a base di erbe: erba di san Giovanni.
- Medicinali usati per la depressione (fluoxetina, paroxetina). Informi il medico di tutti i medicinali che sta usando perché alcuni se combinati con idrossiclorochina potrebbero indurre un danno ai reni, al fegato, alla pelle, al sangue o agli occhi.
Plaquenil con cibi e bevande
- Eviti di prendere il succo di pompelmo in quanto può aumentare il rischio di effetti indesiderati.
Gravidanza e allattamento
Se è in corso una gravidanza, se sospetta o sta pianificando una gravidanza, o se sta allattando con latte
materno chieda consiglio al medico o al farmacista prima di usare questo medicinale. Il medico discuterà
con lei se Plaquenil è adatto a lei.
Gravidanza
L’idrossiclorochina attraversa la placenta. I dati relativi all’uso dell’idrossiclorochina durante la
gravidanza sono limitati.
Plaquenil può essere associato ad un piccolo aumento del rischio di malformazioni, pertanto, non deve
essere usato in gravidanza a meno che il medico consideri che i benefici superino i rischi.
Allattamento
Se lei sta allattando al seno deve fare particolare attenzione se è in trattamento con Plaquenil, poiché
il medicinale è presente in piccole quantità nel latte materno, ma i bambini sono molto sensibili agli effetti
tossici del medicinale. Le pazienti che assumono clorochina devono quindi consultare il medico prima
di allattare al seno.
Guida di veicoli e utilizzo di macchinari
Durante il trattamento con Plaquenil, è sconsigliata la guida e l’uso di macchinari, in quanto
idrossiclorochina può portare ad alterazioni della vista (influenza negativa dell’accomodazione visiva,
annebbiamento della vista). In caso di assoluta necessità parli con il medico che potrà ridurre
temporaneamente la dose.
Plaquenil contiene lattosio
Se il medico le ha diagnosticato un’intolleranza ad alcuni zuccheri, lo contatti prima di prendere questo
medicinale.
3. How to take Plaquenil
Take this medicine following exactly the instructions of your doctor or pharmacist. If you have
any doubts, consult your doctor or pharmacist.
Adults
The recommended dose is:
- for rheumatoid arthritis :initial dose: from 400 to 600 mg per day (from 2 to 3 coated tablets) administered with meals or with a glass of milk. In some patients, the appearance of undesirable effects may require a reduction of the initial dose. Afterwards, after 5-10 days, the dose may be gradually increased to reach the optimal one, often without the undesirable effects reappearing. Maintenance dose: when a good therapeutic response is obtained, usually between 4 and 12 weeks, the dose is reduced to 200 - 400 mg (1 or 2 coated tablets) per day.
- for lupus erythematosus: average initial dose: 400 mg one or two times a day. This dose may be continued for several weeks or months based on your response to the medicine. Maintenance dose: often a dose of 200 to 400 mg per day will be sufficient.
The medicine works by accumulating in the body and several weeks are necessary for the
first beneficial effects to appear, while mild disturbances may occur relatively soon.
Several months of treatment may be necessary before maximum effects can be obtained.
Your doctor will stop Plaquenil if your condition does not improve within 6 months of therapy.
Exceeding the doses described for the maintenance therapy of rheumatoid arthritis and lupus
erythematosus causes a higher incidence of retinal alterations of the eye (retinopathy).
When taking Plaquenil, your doctor may decide to reduce the doses of other medicines you are taking
to control the disease (corticosteroids and salicylates) or to discontinue them completely after Plaquenil has
been taken for several weeks.
If you are taking medicines called corticosteroids, your doctor will gradually reduce your dose:
strictly adhere to your doctor's instructions.
Children
The doctor will adopt the minimum effective dose of Plaquenil without ever exceeding the dose of 6.5 mg per kilo
of weight, per day. Therefore, the 200 mg tablets are not suitable for use in children with a
body weight less than 31 kg. The doctor will still provide the correct instructions also to assess
the correct weight (ideal weight) of the child.
If you take more Plaquenil than you should
Taking an excessive dose of Plaquenil is particularly dangerous in children in whom even
low doses (1 or 2 g) can be fatal.
If you have accidentally ingested an excessive dose of Plaquenil, immediately notify your doctor or
go to the nearest hospital.
Plaquenil is rapidly and completely absorbed by the body after ingestion and in case of
accidental overdose (more rarely even at normal doses if you are a hypersensitive subject) within 30
minutes toxic symptoms such as headache, drowsiness, visual disturbances, severe
decrease in blood pressure (cardiovascular collapse), convulsions, decrease in
blood potassium (hypokalemia) and disturbances of the heartbeat (prolongation of the QT interval,
torsades de pointes, ventricular tachycardia, ventricular fibrillation and other abnormalities
of the electrocardiogram) may appear, which in the most severe cases may be followed by sudden arrest of breathing
and heart, with risk to life. It is necessary to go to your doctor or the nearest hospital
immediately as these effects can occur shortly after taking a
excessive dose.
If you have any doubts about the use of Plaquenil, consult your doctor or pharmacist.
If you forget to take Plaquenil
Do not take a double dose to make up for a missed dose.
If you stop taking Plaquenil
If you have any doubts about the use of this medicine, consult your doctor or pharmacist.
If you should experience a relapse after stopping therapy with Plaquenil, the medicine may
be resumed continuing with administration at intervals, if there are no contraindications
for eye damage.
4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody gets them.
Stop taking Plaquenil and see your doctor immediately if you notice any of the following serious side effects (the frequency cannot be determined based on available data); you may need urgent medical treatment:
- Severe skin reactions (see section 2 Warnings and precautions) such as:
- skin rash with fever and flu-like symptoms and swollen lymph nodes. This could be a condition called Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS)
- skin rash with blisters, widespread scaly skin, pus-filled spots together with fever. This could be a condition called acute generalized exanthematous pustulosis (AGEP)
- skin rash with blisters or peeling of the skin around the lips, eyes, mouth, nose and genitals, flu-like symptoms and fever. This could be a condition called Stevens-Johnson syndrome (SJS)
- multiple skin lesions, itchy skin, joint pain, fever and general feeling of illness. This could be a condition called toxic epidermal necrolysis (TEN)
- skin reaction, including painful, raised, plum-colored sores, especially on the arms, hands, fingers, face and neck, which may also be accompanied by fever. This could be a condition called Sweet syndrome.
- Liver problems: symptoms may include a general feeling of unwellness, with or without jaundice (yellowing of the skin and eyes), dark urine, nausea, vomiting and/or abdominal pain. Rare cases of liver failure (sometimes fatal) have been observed.
The following side effects have also been observed:
Very common side effects(may affect more than 1 in 10 people):
- nausea
- abdominal pain. These symptoms usually resolve quickly by reducing the dose or stopping treatment.
Common side effects(may affect up to 1 in 10 people)
- loss of appetite (anorexia)
- mood instability (emotional lability)
- headache
- blurred vision (due to accommodation disorders that are dose-dependent and therefore transient)
- diarrhea, vomiting that resolves quickly by reducing the dose or stopping treatment
- skin disorders (rash), itching; these effects resolve quickly by stopping treatment.
Uncommon side effects(may affect up to 1 in 100 people):
- nervousness
- dizziness
- changes in a part of the eye called the retina (retinopathy), with vision disorders (changes in pigmentation and visual field defects). In its early stages, the alteration to the retina can heal with discontinuation of therapy, subsequently the risk of worsening is possible even after the end of treatment. Initially, retinopathy may not cause discomfort or cause some visual disturbances (central or near-central vision loss, paracentral and pericentral scotomas, temporal scotomas, altered color perception)
- vertigo, ringing or buzzing in the ears (tinnitus)
- altered values of blood tests for liver function
- changes in the color of the skin and mucous membranes, whitening of the hair, hair loss; these effects resolve quickly by stopping treatment
- alteration of sensitivity and movements (sensory-motor disorders).
Side effects with unknown frequency(frequency cannot be estimated based on available data)
- reduction, even severe, in the number of blood cells (bone marrow depression, anemia, aplastic anemia, agranulocytosis, leukopenia, thrombocytopenia)
- destruction of red blood cells (hemolysis in subjects with a disease called glucose-6-phosphate dehydrogenase deficiency)
- urticaria
- swelling of the face, mouth and throat mucous membranes, with difficulty breathing (angioedema)
- narrowing of the bronchi with difficulty breathing (bronchospasm)
- low blood sugar levels (hypoglycemia)
- worsening of the alteration of some substances that are needed for blood production (porphyria), which can lead to disorders in numerous organs, including the skin
- mental illnesses (psychosis), feeling of depression, thoughts of self-harm or suicide
hallucinations, nervousness or anxiety, feeling of confusion, agitation, difficulty sleeping,
euphoria or hyperexcitability (see section 2)
- irritability
- involuntary eye movements (nystagmus)
- hearing loss
- convulsions
- difficulty with muscle coordination (ataxia)
- changes at the level of the eye (maculopathies and macular degeneration) that may not heal
- heart rhythm disturbances (prolongation of the QT interval)
- heart alteration (cardiomyopathy), which can lead to difficulty in its functioning (heart failure) and in some cases to death, heart rhythm disturbances (bundle branch block/atrioventricular block) and increased volume of a part of the heart (biventricular hypertrophy). Stopping treatment with Plaquenil may lead to healing
- very serious liver disease (fulminant hepatic failure)
- various skin disorders (urticarial, morbilliform, lichenoid, maculo papular, purpura, annular centrifugal erythema)
- worsening of a skin disease called psoriasis, accompanied by fever and increased white blood cells (hyperleukocytosis)
- damage to muscles (musculoskeletal or neuromusculopathies) with weakness and thinning of the muscles. These disorders can heal after discontinuation of treatment, but healing can take months
- slowing of the muscle response to some stimuli and alterations of the nervous system (depression of tendon reflexes and abnormal nerve conduction)
- extrapyramidal disorders, resulting from alterations of a part of the nervous system, which can manifest as muscle contractions, involuntary movements, tremors
- accumulation of a type of fat in the tissues that causes damage (see section 2 Warnings and Precautions). Your doctor may decide to stop treatment with Plaquenil.
Other effects:
- weight loss
- fatigue
- a skin disease called psoriasis not sensitive to light
- changes in a part of the eye called the cornea, a transparent membrane that covers the front of the eye, which include swelling (edema) and opacity, which may be asymptomatic or may cause disturbances such as halos, blurred vision or particular sensitivity to light (photophobia). These disorders may be transient or heal after discontinuation of treatment.

If you get any side effects, even those not listed in this leaflet, tell your
doctor or pharmacist. You can also report side effects directly through the national

By reporting side effects you can help provide more information on the safety of
this medicine.
5. How to store Plaquenil
This medicine does not require any particular storage conditions.
Keep this medicine out of the sight and reach of children.
The expiry date refers to the product in intact packaging, correctly stored.
Do not use this medicine after the expiry date which is stated on the pack after “Exp”. The date
of expiry refers to the last day of that month.
Do not dispose of any medicine in wastewater or household waste. Ask your pharmacist how to
dispose of medicines you no longer use. This will help protect the environment.
6. Contents of the pack and other information
In one tablet:
- The active ingredient is: hydroxychloroquine sulfate 200 mg
- The other ingredients are: lactose monohydrate, povidone, corn starch, magnesium stearate, opadry OY-L-28900 (hypromellose, macrogol 400, titanium dioxide, lactose monohydrate).
Description of the appearance of Plaquenil and contents of the pack
Plaquenil is presented in the form of coated tablets of 200 mg.
One box contains 30 coated tablets.
