1. What is Lopridam and what is it used for

Lopridam is a combination of three active ingredients: perindopril, indapamide and amlodipine. It is a
antihypertensive medicine and is used in the treatment of high blood pressure (hypertension).
Lopridam is used to treat high blood pressure in patients whose blood pressure is already
controlled with the combination of perindopril/indapamide and amlodipine, taken at the same dosage.
Each active ingredient reduces blood pressure and, together, they work to control your blood
pressure:
Perindopril belongs to a class of drugs called ACE inhibitors. These drugs work by widening the
blood vessels; this makes it easier for your heart to pump blood through them.
Indapamide is a diuretic. Diuretics increase the amount of urine produced by the kidneys. However,
indapamide differs from other diuretics in that it causes only a slight increase in the amount of urine produced.
Amlodipine belongs to a class of drugs called calcium antagonists. In patients with high blood
pressure, this medicine works by relaxing the blood vessels so that it is easier
for blood to pass through them.

2. What you need to know before taking Lopridam

Do not take Lopridam

  • if you are allergic to perindopril or to any other ACE inhibitor, to indapamide, to any other sulfonamide, to amlodipine or to other calcium channel blockers, or to any of the other components of this medicine (listed in section 6).
  • if you have had symptoms such as wheezing, swelling of the face or tongue, intense itching or severe skin rashes related to a previous treatment with an ACE inhibitor or if you have experienced such symptoms in any other circumstance (a condition called angioedema).
  • if you have taken or are currently taking sacubitril/valsartan, a medicine used to treat a type of long-term (chronic) heart failure in adults, as this increases the risk of angioedema (rapid swelling under the skin in areas such as the throat).
  • if you suffer from diabetes or your kidney function is impaired and you are being treated with a medicine that lowers blood pressure, containing aliskiren.
  • if you have a severe liver disease or if you suffer from a condition called hepatic encephalopathy (a degenerative brain disease).
  • if you have a severe kidney disease in which the blood supply to the kidneys is significantly reduced (renal artery stenosis).
  • if you are on dialysis or undergoing any other type of blood filtration. Depending on the machine used, Lopridam may not be suitable for you.
  • if you have low or high potassium levels in your blood.
  • if you suspect you have decompensated heart failure (severe fluid retention, difficulty breathing).
  • if you have a narrowing of the aortic heart valve (aortic stenosis) or cardiogenic shock (a condition in which your heart is unable to provide enough blood to the body).
  • if you suffer from heart failure following a heart attack.
  • if you have severe low blood pressure (hypotension).
  • if you are more than three months pregnant (it is best to avoid Lopridam even in the early stages of pregnancy - see “Pregnancy and breastfeeding”).

Warnings and precautions
Talk to your doctor or pharmacist before taking Lopridam.
You must tell your doctor if you have or have had any of the following conditions:

  • aortic stenosis (narrowing of the main blood vessel originating from the heart) or hypertrophic cardiomyopathy (disease of the heart muscle) or renal artery stenosis (narrowing of the artery supplying blood to the kidneys).
  • recent heart attack.
  • heart failure or any other heart problem.
  • problems with your kidneys or are on dialysis.
  • if you have abnormally elevated levels of a hormone called aldosterone in your blood (primary aldosteronism).
  • severe high blood pressure (hypertensive crisis).
  • liver disease.
  • a connective tissue disease (skin disease) such as systemic lupus erythematosus or scleroderma.
  • atherosclerosis (hardening of the arteries).
  • hyperparathyroidism (overactive parathyroid gland).
  • gout.
  • diabetes.
  • if you are following a diet that restricts salt intake or use salt substitutes containing potassium.
  • if you suffer from muscle disorders including muscle pain, tenderness, weakness or cramps.
  • if you are taking lithium or potassium-sparing drugs (spironolactone, triamterene) or potassium supplements, as their use with Lopridam should be avoided.
  • if you are elderly and your dosage may need to be increased.
  • if you have a decrease in vision or eye pain. These could be symptoms of fluid accumulation in the vascular layer of the eye (choroidal effusion) or an increase in pressure in the eye and can occur within a period ranging from a few hours to weeks after taking Lopridam. This condition can lead to permanent vision loss if left untreated. If you have previously had an allergy to penicillin or sulfonamides, you may be at high risk of developing it.
  • if you have had photosensitivity reactions.
  • if you have a severe allergic reaction with rapid swelling under the skin in areas such as the face, lips, mouth, tongue or throat that can cause difficulty swallowing or breathing (angioedema). This can occur at any time during treatment. If you develop such symptoms, you should stop treatment and consult a doctor immediately. See also section 4.
  • if you are taking any of the following medicines, increases the risk of angioedema:
  • racecadotril, a medicine used to treat diarrhea.
  • medicines used to prevent rejection of transplanted organs and to treat cancer (such as temsirolimus, sirolimus, everolimus and other medicines belonging to the class of mTOR inhibitors).
  • vildagliptin, a medicine used to treat diabetes.
  • if you are of black ethnicity as you may have a higher incidence of angioedema and this medicine may be less effective in reducing your blood pressure compared to patients of non-black ethnicity.
  • if you are a hemodialysis patient with high-flux membranes.
  • if you are taking any of the following medicines used to treat high blood pressure:
  • an angiotensin II receptor blocker (ARB) (also known as sartans - for example valsartan, telmisartan, irbesartan), especially if you have kidney problems related to diabetes.
  • aliskiren. The doctor may check your kidney function, blood pressure, and the amount of electrolytes (e.g. potassium) in your blood at regular intervals.

You must tell your doctor if you think you may be pregnant (or if there is a possibility of becoming pregnant). Lopridam is not recommended in early pregnancy, and should not be taken if you are pregnant for more than 3 months, as it can cause serious harm to the baby if taken during this period (see section “Pregnancy and breastfeeding”).
If you are taking Lopridam, inform your doctor or medical staff:

  • if you need to undergo anesthesia and/or surgery.
  • if you have recently had diarrhea or vomiting, or if you are dehydrated.
  • if you need to undergo dialysis or LDL apheresis (blood purification from cholesterol by means of a machine).
  • if you need to undergo a desensitization treatment to reduce the effects of allergies to bee or wasp stings.
  • if you need to undergo a medical examination that requires the injection of an iodine-containing contrast agent (a substance that makes organs such as kidneys or stomach visible on X-rays).

Athletes should know that Lopridam contains an active ingredient (indapamide) that can give a positive result in doping tests.

Children and adolescents
Lopridam should not be given to children and adolescents.

Other medicines and Lopridam
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.
You must not take Lopridam with:

  • lithium (used to treat mania or depression).
  • aliskiren (a medicine used to treat hypertension) if you have diabetes mellitus or kidney problems.
  • potassium supplements (including salt substitutes), potassium-sparing diuretics and other medicines that may increase the amount of potassium in your blood (such as trimethoprim and cotrimoxazole for treating bacterial infections; cyclosporine, an immunosuppressant used to prevent organ rejection; and heparin, a medicine used to thin the blood and prevent the formation of clots).
  • estramustine (used in oncological therapy).
  • other medicines used to treat high blood pressure: ACE inhibitors and angiotensin receptor blockers.
  • sacubitril/valsartan - used for the treatment of a type of long-term (chronic) heart failure in adults (see also the information under "Do not take Lopridam").

Treatment with Lopridam can affect or can be affected by other medicines. Make sure to tell your doctor if you are taking any of the following medicines. The doctor may need to adjust the dose and/or take other precautions:

  • other medicines for the treatment of high blood pressure, including an angiotensin II receptor blocker (ARB) or aliskiren (see also “Do not take Lopridam” and “Warnings and precautions”),
  • potassium-sparing diuretics used in the treatment of heart failure: eplerenone and spironolactone at doses between 12.5 mg and 50 mg per day,
  • anesthetics,
  • iodine contrast agents,
  • moxifloxacin, sparfloxacin (antibiotics: medicines used to treat infections),
  • methadone (used for the treatment of dependencies),
  • procainamide (for the treatment of irregular heartbeat),
  • allopurinol (for the treatment of gout),
  • mizolastine, terfenadine or astemizole (antihistamines for hay fever or allergies),
  • corticosteroids used to treat various conditions including severe asthma and rheumatoid arthritis,
  • immunosuppressants, such as tacrolimus, (used to control the body's immune response, allowing the body to accept the transplanted organ),
  • gold salts for intravenous administration (used to treat rheumatoid polyarthritis),
  • medicines for the treatment of cancer,
  • alofrantine (used to treat some types of malaria),
  • pentamidine (used for the treatment of pneumonia),
  • gold salts for intravenous administration (used to treat rheumatoid arthritis),
  • vincamine (used to treat symptomatic cognitive disorders in the elderly including memory loss),
  • bepridil (used in the treatment of angina pectoris),
  • sultopride (for the treatment of psychosis),
  • medicines for the treatment of heart rhythm problems (e.g. quinidine, hydroquinidine, disopyramide, amiodarone, sotalol),
  • cisapride, difemanil (used to treat stomach and digestive problems),
  • digoxin and other cardiac glycosides (for the treatment of heart problems),
  • baclofen (used to treat muscle stiffness in diseases such as multiple sclerosis),
  • medicines for the treatment of diabetes such as insulin, metformin or gliptins (such as vildagliptin),
  • calcium including calcium supplements,
  • stimulant laxatives (e.g. senna),
  • non-steroidal anti-inflammatory drugs (e.g. ibuprofen) or high doses of salicylates (e.g. aspirin),
  • amphotericin B for injection (for the treatment of severe fungal diseases),
  • medicines for the treatment of mental health disorders such as depression, anxiety, schizophrenia (e.g. tricyclic antidepressants, antipsychotics),
  • tetracosactide (for the treatment of Crohn's disease),
  • medicines, which are more often used to treat diarrhea (racecadotril) or prevent rejection of transplanted organs (sirolimus, everolimus, temsirolimus and other drugs belonging to the so-called mTOR inhibitors class). See the "Warnings and precautions" section,
  • medicines used to treat low blood pressure, shock or asthma (e.g. ephedrine, norepinephrine or adrenaline),
  • ketoconazole, itraconazole (antifungal medicines),
  • ritonavir, indinavir, nelfinavir (so-called protease inhibitors used to treat HIV),
  • rifampicin, erythromycin, clarithromycin (antibiotics: for treating infections caused by bacteria),
  • hypericum perforatum(St. John's wort),
  • verapamil, diltiazem (heart medicines),
  • dantrolene (infusion for severe body temperature abnormalities),
  • simvastatin (medicine to lower cholesterol).

Lopridam with food and drink
People taking Lopridam should not consume grapefruit juice or grapefruit. This is because grapefruit and its juice can induce an increase in the levels of the active ingredient amlodipine in the blood, which can lead to an unquantifiable increase in the blood pressure lowering effect of Lopridam.

Pregnancy and breastfeeding
If you are pregnant, suspect you are pregnant or are planning to become pregnant, ask your doctor or pharmacist for advice before taking this medicine.
Pregnancy
You must tell your doctor if you think you may be pregnant (or if there is a possibility of becoming pregnant). As a rule, your doctor will advise you to stop taking Lopridam before becoming pregnant or as soon as you find out you are pregnant and will advise you to take another medicine instead of Lopridam.
Lopridam is not recommended in early pregnancy, and should not be taken if you are pregnant for more than 3 months, as it can cause serious harm to your baby if taken after the third month of pregnancy.
Breastfeeding
Lopridam is not recommended if you are breastfeeding.
Tell your doctor if you are breastfeeding or are planning to start breastfeeding.
Seek immediate medical attention.

Driving and using machines
Lopridam can affect your ability to drive or operate machinery. If the tablets make you feel unwell, dizzy or tired, or give you a headache, avoid driving or operating machinery and contact your doctor immediately.

Lopridam contains sodium
This medicine contains less than 1 mmol of sodium (23 mg) per tablet, i.e. essentially "sodium-free".

3. How to take Lopridam

Use this medicine following exactly the instructions of your doctor or pharmacist. If you have any doubts
consult your doctor or pharmacist.
The recommended dose is one tablet a day.
Your doctor may decide to modify the dosage regimen if you suffer from impaired
renal function.
Take your tablet with a glass of water, preferably in the morning and before a meal.

If you take more Lopridam than you should
If you take too many tablets, contact the nearest hospital emergency room or inform your
doctor immediately. Taking too many tablets may cause a reduction in your blood pressure, even
dangerously excessive. You may feel dizzy, lightheaded, faint or weak. If the blood pressure drop
is severe enough, shock may occur. Your skin may become cold and sweaty and you may lose
consciousness.
Excess fluid may accumulate in the lungs (pulmonary edema) causing shortness of breath that may
develop up to 24-48 hours after ingestion.
If you have taken an excessive number of Lopridam tablets, contact your doctor immediately .
If you forget to take Lopridam

It is important that you take the medicine every day as regular treatment is more effective.
However, if you forget to take a dose of Lopridam, take the next dose as usual. Do not
take a double dose to make up for a missed dose.

If you stop taking Lopridam
Since treatment with Lopridam for high blood pressure is usually lifelong, you should talk to your
doctor before stopping taking this medicine. Your condition may return if you
stop using this medicine before you have been advised to.
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them.
manifest them.

Stop taking the medicine and consult a doctor immediately if any of the following side effects occur which may be serious:

  • sudden wheezing, chest pain, shortness of breath or difficulty breathing (uncommon) (can affect up to 1 in 100 people),
  • swelling of the eyelids, face, lips, mouth, tongue or throat causing great difficulty in breathing (angioedema) (see section 2 "Warnings and precautions") (uncommon) (can affect up to 1 in 100 people),
  • severe dizziness or fainting due to low blood pressure (common) (can affect up to 1 in 10 people),
  • cardiovascular disorders - irregular heartbeat, angina pectoris (chest, jaw and back pain, triggered by exertion), heart attack (very rare) (can affect up to 1 in
    • 10,000),
  • unusually fast or irregular heartbeat that is life-threatening (torsade de pointes) (frequency not known) (frequency cannot be estimated based on available data),
  • severe skin reactions including skin rashes that often start with itchy red patches on the face, arms or legs (erythema multiforme), intense skin redness, hives, redness of the skin all over the body, severe itching, blisters, peeling and swelling of the skin, inflammation of the mucous membranes (Stevens Johnson syndrome, toxic epidermal necrolysis) or other allergic reactions (very rare) (can affect up to 1 in 10,000),
  • weakness of the arms or legs, or speech problems which could be a sign of a possible stroke (very rare) (can affect up to 1 in 10,000),
  • inflammation of the pancreas which can cause severe abdominal and back pain accompanied by a feeling of severe discomfort (very rare) (can affect up to 1 in 10,000),
  • yellowing of the skin or eyes (jaundice) which could be a sign of hepatitis (very rare) (can affect up to 1 in 10,000),
  • brain disease caused by liver disease (hepatic encephalopathy) (frequency not known) (frequency cannot be estimated based on available data),
  • muscle weakness, cramps, tenderness or pain and in particular, if at the same time you do not feel well or have a high temperature, this may be caused by abnormal muscle breakdown (rhabdomyolysis) (not known).

Other possible side effects:
Since Lopridam is a combination of three active ingredients, the side effects that have been reported are
related to the use of the perindopril/indapamide association or to the use of amlodipine.

Side effects related to the use of perindopril/indapamide
Common
(can affect up to 1 in 10 people):

  • skin reactions in subjects predisposed to allergic and asthmatic reactions,
  • headache,
  • dizziness,
  • vertigo,
  • tingling,
  • vision disturbances,
  • ringing in the ears (tinnitus),
  • cough, shortness of breath (dyspnea),
  • feeling unwell (nausea), feeling sick (vomiting), abdominal pain, taste disturbances, indigestion or difficulty digesting, diarrhea, constipation,
  • skin reactions such as rashes, itching,
  • low levels of potassium in the blood,
  • muscle cramps,
  • feeling weak.

Uncommon(can affect up to 1 in 100 people):

  • mood changes,
  • sleep disturbances,
  • depression,
  • urticaria,
  • red spots on the skin (purpura),
  • clusters of blisters on the skin,
  • kidney problems,
  • impotence (inability to get or maintain an erection),
  • sweating,
  • an excess of eosinophils (a type of white blood cells),
  • changes in laboratory parameters: high levels of potassium in the blood reversible upon discontinuation of treatment, low levels of sodium in the blood which can lead to dehydration and low blood pressure,
  • drowsiness,
  • fainting,
  • feeling your heartbeat (palpitations),
  • fast heartbeat (tachycardia),
  • very low blood sugar levels (hypoglycemia) in diabetic patients,
  • inflammation of blood vessels (vasculitis),
  • dry mouth,
  • increased sensitivity of the skin to the sun (photosensitivity reactions),
  • joint pain (arthralgia),
  • muscle pain (myalgia),
  • chest pain,
  • malaise,
  • peripheral edema,
  • fever,
  • increased plasma urea, increased plasma creatinine,
  • falls.

Rare(can affect up to 1 in 1,000 people):

  • dark urine, feeling unwell (nausea) or feeling sick (vomiting), muscle cramps, confusion and convulsions. These symptoms may be due to a condition called SIADH (syndrome of inappropriate antidiuretic hormone secretion),
  • flushing,
  • worsening of psoriasis,
  • reduced or absent urine production,
  • acute kidney failure,
  • changes in laboratory parameters: increased levels of liver enzymes, high level of serum bilirubin,
  • low chloride content in the blood,
  • low magnesium content in the blood,
  • fatigue.

Very rare(can affect up to 1 in 10,000 people):

  • decreased number of platelets in the blood which can determine unusual bruising or a tendency to bleed,
  • decreased number of white blood cells,
  • decreased red blood cells (anemia),
  • low levels of hemoglobin in the blood,
  • high levels of calcium in the blood,
  • abnormal liver function,
  • confusion,
  • eosinophilic pneumonia (a rare type of pneumonia),
  • stuffy or runny nose (rhinitis).

Frequency not known(frequency cannot be estimated based on available data):

  • abnormal ECG heart tracing,
  • changes in laboratory parameters: high levels of uric acid and sugars in the blood,
  • muscle weakness,
  • myopia, blurred vision,
  • decreased vision and/or eye pain due to high pressure (possible signs of fluid accumulation in the vascular layer of the eye (choroidal effusion) or acute angle-closure glaucoma),
  • if you suffer from systemic lupus erythematosus (a connective tissue disease), this may worsen,
  • abnormal color, numbness and pain in the fingers of the hands or feet (Raynaud's phenomenon).

Blood disorders, kidney, liver or pancreas disorders and abnormal laboratory parameters
can occur. The doctor may decide to perform blood tests to assess
your health status.

Side effects related to the use of amlodipine
Very common
(can affect more than 1 in 10 people):

  • water retention (edema).

Common(can affect up to 1 in 10 people):

  • shortness of breath,
  • headache, dizziness, drowsiness (especially at the beginning of treatment),
  • feeling your heartbeat (palpitations), flushing,
  • abdominal pain, feeling unwell (nausea),
  • change in bowel habits, diarrhea, constipation, indigestion,
  • fatigue, feeling weak,
  • visual disturbances, double vision,
  • muscle cramps,
  • swelling of the ankles.

Uncommon(can affect up to 1 in 100 people):

  • mood changes, anxiety, depression, insomnia,
  • tremors, taste disturbances, fainting,
  • feeling of numbness or tingling in the limbs, loss of pain sensation,
  • feeling of noise in the ears (tinnitus),
  • irregular heartbeat,
  • stuffy or runny nose (rhinitis),
  • cough,
  • dry mouth, feeling sick (vomiting),
  • hair loss, increased sweating, itching, skin rash, red spots on the skin (purpura), skin discoloration, hives,
  • problems with urination, increased need to urinate at night, increased frequency of urination,
  • inability to get an erection, discomfort or enlargement of the breasts in men,
  • pain, feeling unwell,
  • joint or muscle pain, back pain, chest pain,
  • weight gain or loss.

Rare(can affect up to 1 in 1,000 people):

  • confusion.

Very rare(can affect up to 1 in 10,000 people):

  • decreased number of white blood cells,
  • decreased platelets in the blood which can cause unusual bruising or easy bleeding,
  • excess sugar in the blood (hyperglycemia),
  • nerve diseases that can cause muscle weakness, tingling or numbness,
  • swelling of the gums,
  • inflammation of the stomach (gastritis),
  • inflammation of the liver (hepatitis), yellowing of the skin (jaundice), increased levels of liver enzymes,
  • increased muscle tension,
  • a skin rash that often starts with itchy red patches on the face, arms and legs (erythema multiforme),
  • inflammation of blood vessels, often with skin rashes,
  • changes in skin appearance after exposure to the sun or artificial UVA (photosensitivity reaction).

Frequency not known(frequency cannot be estimated based on available data):

  • tremors, postural rigidity, mask-like face, slow movements and shuffling gait.

If you get any side effects, even those not listed in this leaflet, tell your doctor

5. How to store Lopridam

Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the package and on the blister after “EXP”.
The expiry date refers to the last day of that month.
This medicine does not require any particular storage temperature.
Store in the original blister to protect the medicine from light and moisture.
Do not dispose of any medicine in waste water or household waste. Ask your pharmacist how to dispose of
medicines you no longer use. This will help protect the environment.

6. Package Contents and Other Information

What Lopridam contains
The active ingredients are perindopril erbumine, indapamide and amlodipine besylate.

Each tablet contains 8 mg of perindopril erbumine equivalent to 6.676 mg of perindopril, 2.5 mg of
indapamide and 5 mg of amlodipine as amlodipine besylate.

Each tablet contains 8 mg of perindopril erbumine equivalent to 6.676 mg of perindopril, 2.5 mg of

The other ingredients are:
microcrystalline cellulose PH-112, anhydrous calcium hydrogen phosphate; red iron oxide (E172); croscarmellose
sodium; colloidal anhydrous silica; magnesium stearate.

Description of the appearance of Lopridam and contents of the pack
Lopridam 4 mg/ 1.25 mg/ 5 mg tablets: round tablets of dark marbled pink color, with a
diameter of 7 mm imprinted with "4 1.25 5" on one side.
Lopridam 4 mg/ 1.25 mg/ 10 mg tablets: round tablets of light pink color, with a diameter of 9.4
mm imprinted with "4 1.25 10" on one side.
Lopridam 8 mg/ 2.5 mg/ 5 mg tablets: round tablets of marbled pink color, with a diameter of
9.4 mm imprinted with "8 2.5 5" on one side.
Lopridam 8 mg/ 2.5 mg/ 10 mg tablets: round tablets of dark marbled pink color, with a
diameter of 9.4 mm imprinted with "8 2.5 10" on one side.
Lopridam is available in OPA/Alu/PVC/Alu blisters.
Packs: 30, 60, 90 tablets.
