1. What is Ankermann and what is it used for
This medicine contains cyanocobalamin, known as vitamin B12.
Most people get enough vitamin B12 with food, but if you have had
stomach surgery or have certain intestinal diseases, or if you follow a
restrictive diet or are taking certain medications, you may not absorb enough vitamin B12.
This medicine is used for the treatment of vitamin B12 deficiency and maintenance therapy in
adult patients:
○ Who have been diagnosed with pernicious anemia.
○ Who have malabsorption of vitamin B12 (e.g. due to atrophic gastritis, inflammatory bowel
disease, celiac disease, total or partial gastrectomy, ileal resection, gastric bypass or other
bariatric surgery).
○ With long-term use of histamine H2 receptor blockers, proton pump inhibitors, aminosalicylates and metformin.
This medicine is also used to treat vitamin B12 deficiency due to reduced dietary intake (e.g. vegetarian diet) in adults.
2. What you need to know before taking Ankermann
Do not take Ankermann
- if you are allergic to cyanocobalamin or vitamin B or to any of the other ingredients
- if you have an anemia caused by an alteration in the production of cells at the level of the bone marrow (megaloblastic anemia, a condition in which the body produces unusually large and immature red blood cells) caused exclusively by a folate deficiency (vitamin B)
- if you suffer from vision disorders caused by abuse of tobacco or alcohol, which is often associated with nutritional deficiencies (nutritional optic neuropathy) or inflammation of the optic nerve (retrobulbar neuritis) that occurs following a disease characterized by a deficiency of
red blood cells (pernicious anemia), or have any other condition that requires the elimination
of cyanide from the body (detoxification). In this situation, different medicines
based on cobalamin must be taken
- if you suffer from degeneration of the optic nerve (which can lead to vision problems as the nerve that carries visual messages from the retina to the brain is damaged)
Warnings and precautions
Talk to your doctor or pharmacist before taking this medicine.
At the beginning of treatment, the doctor must make a diagnosis to establish the underlying cause of the
deficiency, including an assessment of the function of the gastrointestinal tract. The doctor will determine if
the absorption of vitamin B from food is insufficient and if oral supplementation with this
medicine is required.
Due to the severity of the disease, the doctor will probably want to monitor your response to
this medicine within the first 3 months. This will likely involve performing a blood test. If you adhere well to the treatment plan, therapy can continue for life, based
on the underlying disease.
If you need periodic kidney dialysis, the doctor must perform regular blood tests and it may be
necessary to reduce the dosage of this medicine.
If you have a folic acid deficiency, this may weaken your response to therapy. In this case,
the use of this medicine must be associated with supplementation with folic acid.
Ankermann contains 1,000 micrograms of cyanocobalamin per tablet. If you are planning to undergo
laboratory tests, you must inform your doctor or laboratory personnel that you are taking or have
recently taken this medicine, because cyanocobalamin can affect the results of such
tests (including blood tests, urine tests, skin tests using allergens, etc.) due to
its red color. This medicine can affect the results of tests for the determination of
anti-intrinsic factor antibodies (IFA).
Children and adolescents
Do not give this medicine to children and adolescents under 18 years of age, as the potential risks and
benefits are still unknown.
Other medicines and Ankermann
Tell your doctor or pharmacist if you are taking, have recently taken or might take
any other medicines, including over-the-counter medicines.
The effects of this medicine may be affected by:
- Proton pump inhibitors (e.g. omeprazole) and histamine H2 receptor antagonists (e.g. cimetidine); both are used to treat excess acid in the stomach that causes indigestion or ulcer
- Colchicine, a treatment for gout
- Aminoglycosides (e.g. neomycin) and chloramphenicol, which are antibiotics
- Biguanides, e.g. metformin, which is used in the treatment of diabetes
- Aminosalicylic acid, which is used to treat inflammatory bowel diseases
- Oral contraceptives
- Antiepileptic drugs (e.g. carbamazepine)
- Potassium salts
- Methyldopa, which is used to treat high blood pressure
- Prolonged use of nitrous oxide (a gas used for anesthesia)
- Second-generation antipsychotics (e.g. olanzapine and risperidone)
- Cholestyramine, which is used to reduce high cholesterol levels in the blood
- Glucocorticoids (e.g. prednisolone), which are often used to treat inflammatory diseases
Ankermann with alcohol
Alcohol can affect the absorption of this medicine into the body.
Pregnancy, breastfeeding and fertility
If you are pregnant, suspect you may be pregnant or are planning to become pregnant, or if you are breastfeeding,
ask your doctor or pharmacist for advice before taking this medicine. As a precaution, it is preferable to avoid the use of this medicine during pregnancy, unless treatment with cyanocobalamin is made necessary by the woman's condition.
This medicine can be used during breastfeeding.
Driving and operating machinery
This medicine should not affect your ability to drive and operate machinery.
Ankermann contains lactose, sucrose, macrogol hydroxystearate and sodium

If you have been told by your doctor that you have an intolerance to some sugars, contact your doctor before
taking this medicine.
Macrogol hydroxystearate
May cause stomach upset and diarrhea.
Sodium
This medicine contains less than 1 mmol (23 mg) of sodium per tablet, i.e. it is essentially
“sodium-free”.
3. How to take Ankermann
Take this medicine following exactly as indicated in this leaflet or as instructed by your doctor or pharmacist. If you have any doubts, consult your doctor or pharmacist.
Use in adults
The recommended daily dose is 1 tablet, which corresponds to 1,000 micrograms of
cyanocobalamin, in the following indications:
- Treatment of vitamin B deficiency in patients with reduced dietary intake (e.g. vegetarian diet).
- Treatment of vitamin B deficiency and maintenance therapy in patients diagnosed with pernicious anemia or who have vitamin B malabsorption
- Treatment in case of long-term use of histamine H2 receptor blockers, proton pump inhibitors, aminosalicylates and metformin.
Patients with severe hematological or neurological symptoms of vitamin B deficiency should be
initially treated parenterally until blood levels are normalized.
If well tolerated, oral vitamin B can be substituted for life in cases of pernicious anemia or
vitamin B malabsorption. The response to therapy should be monitored regularly. See
also section 2. Warnings and precautions.
If you have kidney problems
In patients with moderate kidney problems, this medicine can be used at the normal dosage. In
case of severe kidney problems, a dose reduction is recommended. In addition, the amount of
vitamin B in the blood serum should be checked regularly.
If you have liver problems
It is not known whether this medicine is safe for people with liver problems. Inform your doctor if you have
liver problems.
How to take the tablets
Ankermann is for oral use. The tablets should be swallowed whole with some water,
preferably in the morning on an empty stomach. The duration of treatment depends on how your
body reacts to the medicine. To find the optimal dose, the doctor must perform regular checks.
If you take more Ankermann than you should
If you take a higher amount of this medicine than you should, contact your doctor, who
will decide on the necessary measures. No cases of intoxication have been reported.
There is no known antidote for overdose. In case of overdose, the symptoms should be
treated.
If you forget to take Ankermann
If you forget to take a dose, take it as soon as you remember you haven't taken it, unless
it is time to take the next dose. In this case, do not take the forgotten dose but take the
usual dose at the right time. Do not take a double dose to make up for a forgotten dose.
If you stop taking Ankermann
Always consult your doctor before stopping treatment.

4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody will experience them.
Uncommon: may affect up to 1 in 100 people
- Severe allergic reactions with symptoms such as hives, rash, or itching in large areas of the body.
Not known (frequency cannot be estimated from available data):
- acne-like eruptions and blisters;
- fever.

5. How to store Ankermann
Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the pack after “Exp.”.
The expiry date refers to the last day of that month.
Do not store above 25 °C.
Do not dispose of any medicine in waste water or household waste. Ask your pharmacist how to
dispose of medicines you no longer use. This will help protect the environment.
6. Contents of the pack and other information
What containsAnkermann
The active ingredient is: 1000 micrograms of cyanocobalamin (a synthetic form of
vitamin B12).
The other ingredients are: lactose monohydrate, povidone K30, sodium croscarmellose, stearic
acid 50 (stearic acid and palmitic acid), white AquaPolish P*,
sucrose, talc, heavy kaolin, calcium carbonate, titanium dioxide,
dried acacia dispersion, macrogol 6000, macrogolglycerol
hydroxystearate 40, sodium lauryl sulfate (E487), glycolated montan wax.

Description of the appearance of Ankermann and contents of the pack
White, round, biconvex coated tablet, with an approximate diameter of 9.8 mm.
Aluminum-PVDC/PVC blisters in cardboard boxes. Packs of 10, 25, 30, 50, 60, 90 and
100 coated tablets.
Not all pack sizes may be marketed.
