COMPOSITION
Each capsule contains:
Active ingredient:Paromomycin sulfate mg 357.2 (equivalent to mg 250 of paromomycin)
Excipients:talc, gelatin, titanium dioxide.

THERAPEUTIC INDICATIONS
Infections of the intestinal tract due to sensitive germs (E. Coli, Shigella, Salmonella with
exclusion of S.typhi etc.). Sterilization of the intestinal contents in the
preparation for interventions on the intestine. Intestinal amebiasis (acute and chronic).
Adjunct in the treatment of hepatic coma (for sterilization of the intestinal flora
producing ammonia).
CONTRAINDICATIONS
Hypersensitivity to the drug. Severe ulcerative or intestinal obstruction even
partial to avoid the risk of absorption higher than normal. Do not administer
the antibiotic simultaneously with other agents potentially nephrotoxic such as for
example: Neomycin, Kanamycin, Streptomycin, Colistin, Viomycin, Gentamicin,
Cephalosporins, Polymyxin. Children under two years of age. Renal insufficiency.
Myasthenic syndromes. Malabsorption syndromes.
SPECIAL PRECAUTIONS FOR USE
Following prolonged administration, development of non-sensitive microorganisms may occur:
in this case the patient must be kept under close control establishing a
suitable therapy. Paromomycin is normally not absorbed by the digestive tract, but
absorption may occur abnormally, and in an unpredictable manner, at the level of
lesions of the intestinal mucosa; therefore, since the antibiotic is potentially oto and
nephrotoxic, it is advisable, especially in long-term therapies, to periodically perform
checks of renal function and audiometric tests. Drugs with anti-kinetic effects should not
be administered simultaneously because these could prevent timely detection of early
signs of ototoxicity. Particular caution is required in the treatment of patients with
kidney disease even of moderate severity in order to detect any accumulation of small
doses of antibiotic which may sometimes be absorbed especially at the level of intestinal
lesions. Similar caution is required in patients with liver disease.
WARNINGS
In pregnant women, the product should be used only in cases of actual necessity,
under the direct control of the doctor. Discontinuation of therapy is recommended
once the symptoms related to the infection have ceased (treatment however should not
be prolonged beyond 3-5 days).
The medicine is not contraindicated for subjects affected by celiac disease.
Keep out of reach of children.
DOSAGE, ADMINISTRATION AND DURATION OF TREATMENT(unless otherwise prescribed):
The dosages, expressed in mg of paromomycin base, for the various indications, are the
following: Bacterial dysentery: 35/50 mg/kg of weight (in adults 2-3 g). It is
advisable to discontinue therapy once the symptoms related to the infection have ceased
(treatment, in general, should not be prolonged beyond 3-5 days).
Amebiasis:25/35 mg/kg of weight, administered in 3 doses with meals, for
5-10 days. In particularly resistant forms, the dose may be increased at
the doctor's discretion.
Pre-operative sterilization of the intestine:35 mg/kg of weight for 4 days.
Sterilization of the intestine during hepatic coma:adults: average dose 4 g per day,
appropriately divided throughout the day, for 5-6 days.
UNDESIRABLE EFFECTS
When the drug is administered at high doses (greater than 3 g per day), the onset of diarrhea, abdominal cramps, feeling of
nausea has been observed in some subjects.
Furthermore, in cases of abnormal absorption, especially with high or excessive doses, cases of
nephrotoxicity (with oliguria, albuminuria, hematuria, etc.) or
ototoxicity (with dizziness, tinnitus, hearing loss) have been reported. Rarely reported reactions of
hypersensitivity with skin rashes of various types or locations.
